We are seeking an Occupational Therapist for our newly established MGH2Home Program - FT - Salary Employee

What will be your role?

This is a unique Occupational Therapist position supporting East Toronto Health Partners (ETHP) strategy to improve client transitions from hospital-to-home through the newly established MGH2Home program. This short-term transitional program (up to 90 days) involves linking together Michael Garron Hospital (MGH), Spectrum Health Care, VHA Home HealthCare and WoodGreen to support earlier discharges of adults with higher needs to reintegrate into their home and community. This multi-disciplinary team will work as one team, along with the client and designated family, to address their complex health and social needs in the development of a single one team care plan.

As an active member of a multidisciplinary team, OTs will provide consultation, assessment and planned interventions for diverse client groups in the community discharged from MGH2Home program. The Occupational Therapist works within a continuous quality improvement framework, which reflects VHAs core beliefs and mission of services as well as promoting and maintaining good public relations with clients, customers and VHA staff, volunteers, and associates.

There is a $2,000 Sign-On Bonus for full-time occupational therapists only.

Required Qualifications:

• Graduate of a recognized program for Occupational Therapy.

• Member of the College of Occupational Therapists of Ontario.

• Membership in professional association preferred.

• Experience within the professional discipline.

• Specialized courses to support areas of clinical expertise.

• Knowledge of community care requirements and resources available.

• Exceptional written and verbal skills.

• Ability to work independently.

• Utilize an evidence-based to achieve client-centered outcomes.

• Familiar with the routine use of valid framework and reliable outcome measures to support clinical interventions.

• Demonstrated commitment to client and family centered care.

• Demonstrated ability to recognize safety concerns, communicate effectively using a computer and/or mobile device.
