Job Summary

The Health Care Aide plays a vital role in providing compassionate and personalized care to patients in various settings, including assisted living facilities, nursing homes, and home health environments. This position involves assisting individuals with daily living activities, monitoring their health status, and ensuring their comfort and safety. The ideal candidate will possess a strong commitment to patient care and have experience working with diverse populations, including those with developmental disabilities and memory-related conditions.

Duties

Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting. Provide support in medication administration and ensure compliance with prescribed treatments. Monitor vital signs and report any changes in patient conditions to the supervising nurse or healthcare provider. Maintain accurate medical documentation and records in accordance with HIPAA regulations. Offer companionship and emotional support to patients, particularly those with Alzheimer's or dementia. Assist with mobility using equipment such as Hoyer lifts when necessary. Perform light housekeeping duties including cleaning, laundry, and meal preparation to maintain a safe living environment for patients. Engage in patient observation to identify any changes in behavior or health status that may require attention. Collaborate effectively with healthcare teams to provide comprehensive care tailored to individual patient needs.

Skills

Proficiency in patient care techniques including CPR and catheter care. Familiarity with EMR/EHR systems for accurate medical documentation. Knowledge of medical terminology relevant to caregiving practices. Experience in long-term care settings or hospice care is preferred. Ability to work compassionately with individuals who have developmental disabilities or require memory care. Strong organizational skills for managing multiple tasks efficiently while ensuring high-quality patient care. Excellent communication skills to interact effectively with patients, families, and healthcare professionals. Capability to prepare nutritious meals catering to dietary restrictions as needed.
