ABOUT US:

Onion Lake Family Services is an organization that strengthens family units by offering support and resources to parents and caregivers, aiming for healthy homes. Our short-term, strength based services focus on empowering families for long term improvement and skill building.

The Ohpikowin Case Manager will provide wholistic wrap-around support to young people who access Post majority Support Services through Onion Lake Ohpikowin Program. The Case Manager will work directly with youth to support transition to independence from child welfare. The Case Manager will aid and guide youth within Onion Lake Cree Nation to find and build resources & services, develop plans and report on processes & standards to ensure positive outcomes and a smooth transition of Ohpikowin. Will keep apprised of child and family services legislation, regulations, and policies & procedures.

Strong understanding of the challenges faced by youth in care and aging out of the foster care system.

Work closely with OLFS Family Worker and youth who are 12 years of age.

Knowledge of OLCN resources.

Compassion, patience and a commitment to empowering youth in their transition to adulthood.

Collaborate with community services and programs from Wellness, Health, Housing, Education, Income assistance & Employment on a regular basis.

Share progress with the youth's circle of care network and create linkages to other supports.

Identify challenges and collaborate with others to find suitable solutions.

Develop individual plans identified by the youth and allow them to make decisions on all matters affecting them.

Work with youth to take the lead in all meetings.

Find life skills support, which includes but are not limited to, learning, educational, professional development opportunities, financial support and safe, stable comfortable housing, physical mental and social well-being, reconnection with land, culture, language and community, gender identity.

Ensure LGBTQI+ youth receive non-discriminatory support and services.

Adhere and enforce OLFS policies and procedures.

Other duties that are complimentary of the Ohpikowin Case Manager Position.

QUALIFICATIONS AND EDUCATION REQUIREMENTS

Social Services Worker Diploma, Bachelor of Social Work Degree or Psychology or related field.

Experience with Cree children, families, people, organizations, and communities.

Valid class 5 driver's license with acceptable abstract

Clear criminal record with vulnerable sector

• Ability in Microsoft Word, Excel, PowerPoint, Desktop applications.
• Respect for, as well as knowledge and understanding of Onion Lake Cree Nation traditional practices and protocols.
• Ability to understand and speak Cree is a definite asset
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Ability to work, plan, coordinate with the youth's circle of care network.
