Position Overview
We are seeking a dedicated
Occupational Therapy Assistant
to support individuals transitioning from hospital to community settings. This dynamic, community-based role focuses on facilitating safe discharges and empowering clients to re-engage in meaningful daily activities within their homes and communities.
Key Responsibilities
Hospital Discharge Support:
Assist Occupational Therapists with coordinating hospital discharge recommendations
Facilitate arrangements for transportation and medical equipment procurement
Provide safety training on medical equipment use under OT supervision
Support implementation of discharge plans in community settings
Community Integration:
Accompany clients to community medical appointments
Document and relay appointment details and outcomes to supervising Occupational Therapist
Support clients in accessing community resources and services
Facilitate smooth transitions between healthcare settings
Activities of Daily Living (ADL/IADL) Support:
Assist clients in re-engaging with personal care activities
Support instrumental activities of daily living in home and community environments
Implement treatment plans developed by supervising Occupational Therapist
Monitor client progress and report observations to OT
Work Environment
Location:
Community-based across the Niagara Area
Setting:
Client homes, community facilities, medical appointments
Population:
Primarily adults with opportunity to work with pediatric clients
Flexibility:
Applicants can select populations they are passionate about supporting
Qualifications
Required:
Graduation from an accredited Occupational Therapy Assistant program
Registration with appropriate regulatory body
Valid driver's license and reliable transportation; able to transport clients in vehicle
Ability to travel throughout the Niagara Area
Strong communication and interpersonal skills
Ability to work independently while maintaining regular supervision contact
Preferred:
Experience in hospital or community healthcare settings
Knowledge of community resources in Niagara
Experience with discharge planning processes (although not required)
CPR/First Aid certification
