CORE RESPONSIBILITIES OF THE POSITION
The Occupational Therapist will provide occupational therapy for seniors living with Chronic Diseases and/or physical disabilities. As a key member of the inter-professional Chronic Disease Management team, the Occupational Therapist will:
Perform clinic and in home assessment, treatment, health education/counselling and other occupational therapy related activities according to the College of Occupational Therapists of Ontario.
Implement appropriate strategies by working with the inter-professional team using a client centered approach.
Act as a client advocate.
Provide Home Safety Assessments including determining placement of grab bars as appropriate.
Assess the client's status in person and recommend the appropriate device to best meet the Applicant's basic needs.
Informs client about all applicable ADP policies, eligibility criteria, and procedures, so that the Applicant and/or Agent can make an informed decision regarding the submission of an application to the Program.
Works with the client's Registered Vendor to enhance their own knowledge of ADP funded equipment by discussing the Applicant's equipment needs and technical.
Contribute to chart documentation, participate in chart reviews and case conferences and make referrals to other community services as appropriate.
Contribute to the ongoing development and implementation of seniors' health services; organizational initiatives that are relevant to the care of seniors and that actively link frail and pre-frail seniors.
Demonstrate a commitment to a safe environment for staff, clients and families by working in compliance with weCHC related policies, health and safety regulations and completing mandated training and educational sessions as required.
SKILLS, KNOWLEDGE & EXPERIENCE REQUIRED
Minimum 3-5 years recent experience in occupational therapy working with the senior population, with preference to those who have worked in a community health setting.
Proficiency in current assessment, and therapeutic design techniques working with seniors.
Thorough knowledge and understanding of community resources specific to the senior's population. .
Strong attention to patient and family-centered care and establishing therapeutic relationships.
Must provide a clean Driver's Abstract. Current, valid Class "G" Driver's License is required and reliable personal transportation to support travel between locations.
Demonstrated ability to work effectively in an inter-professional team environment.
Strong communication and excellent interpersonal skills, combined with good analytical, problem solving, organizational and administrative skills.
Bilingual (English/French) language skills are an asset.
Proficiency in the use of computers and various software applications.
Undergraduate degree in Occupational Therapy from a recognized University.
Member in good standing with the College of Occupational Therapists of Ontario.
Completion of a Vulnerable Sector check prior to commencing employment.
