Your Opportunity:Rehabilitation Navigator- Pediatrics- Neuromotor/Neurodevelopmental. Please note, this position is open to Allied Health Professionals- Physiotherapists, Occupational Therapists, Recreation Therapists, Social Workers, Speech Pathologists, and Psychologists. The role of the Rehabilitation Navigator is to manage referrals and assist patients (which includes all Glenrose pediatric patients aged 0-18 years) and their families with navigation between ambulatory clinics, Specialized Rehabilitation Outpatient Program (SROP-Peds), and inpatient services. The Rehabilitation Navigator will identify and coordinate services that are most appropriate. The RNav will ensure patients meet eligibility criteria; collaborates with families to identify and resolve barriers to receiving treatment; provides orientation and education regarding the SROP service delivery model; organizes and facilitates team and family conferences (including virtual health, i.e., when arranging patient appointments, to provide updates on treatment status, and to arrange or modify future treatment plans); develops relationships with key service providers and stakeholders to facilitate continuity and coordination of care for patients and families; shares timely and appropriate information among team and other clinics/services/programs to facilitate a shared understanding of family\'s needs and priorities; supports the provision and coordination of care in rehabilitation service for families from rural communities and Canada\'s north. The Rehabilitation Navigator will optimize the use of outpatient inter-professional and community resources to support access to family centered, goal directed services to maximize their functional outcomes.Description:As an Occupational Therapist II, you will be responsible for the coordination of occupational therapy and other allied health clinical services within a program or service area. You will provide supervision of students and team members as assigned. As an Occupational Therapist II, you will be responsible to work both independently and as part of an interdisciplinary team where you will be an integral member, providing leadership to other healthcare professionals. You will be making day-to-day clinical decisions on patient/family needs regarding assessment, intervention, discharge and follow-up. You will make decisions and advise team members on complex clinical cases and provide clinical leadership within the program or service area to ensure safe, efficient and effective client care. You will ensure occupational therapy practice is evidence-based and in alignment with Alberta legislation and regulations, professional standards of practice, code of ethics and AHS policy and guidelines. You will be providing coordination of the day-to-day operations of an assigned service area and monitoring care delivery, identifying service needs and assisting other medical staff with caseload management, completing chart audits and providing follow-up and feedback to staff. You will be participating in program development and operational planning and representing the department on committees or programs. You will be making decisions on staffing level requirements, workflow and caseload priorities in collaboration with managers and other healthcare providers. As assigned, you will carry out the roles of a direct clinical service provider, consultant, educator and/or advocate in your program or service area.
