Your Opportunity:As the Team Lead, Occupational Therapist II, you will play a crucial role in providing leadership and expertise to support two chronic disease management programs: Occupational Therapy Services for Chronic Disease Management and Supervised Transitional Exercise Program. Occupational Therapy Services for Chronic Disease Management offers integrated occupational therapy services to support clients and/or caregivers to identify functional challenges, establish and take action to achieve meaningful goals. Supervised Transitional Exercise Program (STEP Forward) is a targeted physical conditioning group program to support clients with chronic health conditions transition into independent community exercise programming. The Occupational Therapist II will play a critical role to sustain, foster and promote the programs with the purpose to support clients to live meaningful, healthy, and safe lives using a behavior change framework approach. The services will meet the specific health needs of clients and families through education in virtual, in-person, and group formats. This program will be delivered from community sites or at client homes as required.Description:As an Occupational Therapist II, you will be responsible for the coordination of occupational therapy and other allied health clinical services within a program or service area. You will provide supervision of students and team members as assigned. As an Occupational Therapist II, you will be responsible to work both independently and as part of an interdisciplinary team where you will be an integral member, providing leadership to other healthcare professionals. You will be making day-to-day clinical decisions on patient/family needs regarding assessment, intervention, discharge and follow-up. You will make decisions and advise team members on complex clinical cases and provide clinical leadership within the program or service area to ensure safe, efficient and effective client care. You will ensure occupational therapy practice is evidence-based and in alignment with Alberta legislation and regulations, professional standards of practice, code of ethics and AHS policy and guidelines. You will be providing coordination of the day-to-day operations of an assigned service area and monitoring care delivery, identifying service needs and assisting other medical staff with caseload management, completing chart audits and providing follow-up and feedback to staff. You will be participating in program development and operational planning and representing the department on committees or programs. You will be making decisions on staffing level requirements, workflow and caseload priorities in collaboration with managers and other healthcare providers. As assigned, you will carry out the roles of a direct clinical service provider, consultant, educator and/or advocate in your program or service area.
