Your Opportunity:

Integrated Home Care uses a strengths-based Client Centered Case Management practice framework. Using a person-centered approach you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals. In collaboration with the client/family and interdisciplinary team you will ensure services are provided and modified on an ongoing basis, based on client\'s response to interventions, incorporating stewardship into service authorization. You will report to the Home Care Manager and will be responsible for coordinating and providing quality care to clients in the community. You will provide assessments, case coordination and direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment. The successful incumbent will be expected to demonstrate proficiency in the following areas: personal and professional accountability, systems thinking, creative thinking, communicating and building relationships, organizational and time management skills, interpersonal skills and intermediate level of computer skills.

Description:

As an Occupational Therapist (OT), you will provide evidence-informed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including self-care, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, self-management, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client\xc2\xbfs physical, institutional, cultural, and social environments. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will collaborate with clients and families in determining, implementing, and evaluating rehabilitation. You will participate in a variety of interprofessional teams to build awareness, enrich support and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for the clients. You will contribute to quality improvement and their professional community and participate in peer mentoring and supervising students. You may also provide clinical supervision to Therapy Assistants and Therapy Aides.

Required Qualifications:

Completion of bachelor\'s degree in Occupational Therapy from an accredited university program. Registered with the Alberta College of Occupational Therapists (ACOT). Additional Required Qualifications:

The candidate must have at least 2 years of occupational therapy experience with one year in a community-based setting (Home Care experience preferred). Must have current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) from Heart and Stroke Foundation within the last 12 months. This role requires travel for work related purposes is a requirement of this position. You will be required to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example: lifting of approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job. Learning resources and training are provided to safely perform work. Preferred Qualifications:

Experience providing case management services.
