We are seeking Nurse Team Leads right across Canada, and two new Team Leads in the Oakville area is the current priority, with two more for the Toronto area in the future, all are full-time permanent jobs.

The Work You Will Do

\xe2\xa6\x81 You will build a trusted partnership with clients as their in-home care provider, following their lead, co-creating a care plan, delivering the highest quality of home care with compassion, clarity, and reliability.

\xe2\xa6\x81 You will provide the care yourself until you can train one of your team members to provide a consistent, ongoing promoting emotional, physical, and overall well-being.

\xe2\xa6\x81 You will arrange for the initial and ongoing assessment of clients requiring licensed and unlicensed care and service as appropriate.

\xe2\xa6\x81 You will create care plans for clients as appropriate; visit clients as required all while following current best practices in nursing.

\xe2\xa6\x81 Liaise with other care providers, including family as well as the public healthcare system.

\xe2\xa6\x81 You may serve as the on-call Nurse if required.

\xe2\xa6\x81 Participate in community outreach and development during at least 20% of your work time.

\xe2\xa6\x81 Lead or assist with the integration of client satisfaction initiatives; ensure that all referred client concerns and complaints are satisfactorily addressed and documented promptly.

\xe2\xa6\x81 You will help to coordinate human resources concerning recruitment and selection, orientation and training, discipline, and the probationary/annual performance review of reporting neighborhood health team members

\xe2\xa6\x81 Supervise and support licensed and unlicensed employees to ensure compliance with all federal and provincial legislated standards of practice as well as company policies and procedures.

\xe2\xa6\x81 You will be responsible for the safety of your team and we ask that you notify your immediate Supervisor of any Health & Safety risks or concerns.

\xe2\xa6\x81 You will be responsible for completing Accident Reports for all direct reports

\xe2\xa6\x81 We expect you to participate in Health & Safety activities while performing all duties.

\xe2\xa6\x81 You will read, interpret and implement relevant policies and procedures.

\xe2\xa6\x81 We will provide you with internal and/or external continuing education activities.

\xe2\xa6\x81 You will be an active participant in quality activities and continuous improvement initiatives in keeping with the company\'s Quality Management System.

\xe2\xa6\x81 Demonstrate knowledge of Standards of Practice guidelines of the Provincial regulating body.

\xe2\xa6\x81 At least five years of recent experience with progressive responsibilities in Nursing, preferably in a community setting

\xe2\xa6\x81 Experience in community outreach

\xe2\xa6\x81 Focus on geriatric care, community health or social work, or an equivalent combination of education and experience would be an asset

Ideal Profile for our Nurse Team Leads

\xe2\xa6\x81 You are an RPN or RN and licensed in Ontario by the CNO.

\xe2\xa6\x81 You are experienced in the home care sector and working in the community.

\xe2\xa6\x81 You have leadership skills to coach your team of about 15 PSWs and other care givers.

\xe2\xa6\x81 Your caring nature shines through as you always ensure that your clients remember how you make them feel because that is what matters the most.

\xe2\xa6\x81 You value teamwork and are ready to be an active partner leading a small local care team.

\xe2\xa6\x81 You earn and keep client trust and treat your clients like you would treat your family

\xe2\xa6\x81 You thrive when you are showing you care via acts of service

\xe2\xa6\x81 You understand how important home care is for those in need.
