Nurse Led Long?Term Care Outreach




Reporting to the Director ? Trauma Program, Emergency & Critical Care Services, the NLOT Nurse Practitioner (NP)
will be responsible for providing quality nursing care to complex patient populations within the community. As part of
an inter professional health care team, you will practice within the Standards of the College of Nurses of Ontario, the
Philosophy and Values of Nursing Services and the policies of the Hospital.


Applicants must have the following qualifications:
Current registration as a Registered Nurse (EC) with the College of Nurses of Ontario Graduate from an approved Nurse Practitioner program Current certification in BCLS Experience working with geriatric populations managing acute and chronic conditions Certification, or recent continuing education related to geriatric nursing A valid Ontario driver's license and access to a reliable vehicle

In addition, applicants must have skill and experience in the following elements of nursing practice:
Physical Assessment Intravenous Therapy and Intravenous Initiation Central Venous Line Management Aseptic Technique and Body Substance Precautions Demonstrated ability to work independently and collaboratively with an inter professional team Ability to educate, apply adult learning principles and evidenced based research into practice Ability to maintain positive patient, family and staff relations Critical thinking, problem solving, prioritization and conflict resolution Ability to self direct, interact, and adapt effectively with other professionals in complex, dynamic situations Ability to communicate and collaborate with patients and families about health findings, treatment options, self care, and advance care planning


In addition to the above qualifications, preference will be given to applicants who have:
Ability to transfer knowledge, teach, coach and mentor others Demonstrated leadership and interprofessional communication skills Demonstrated computer skills Knowledge and experience working with residents in Long? Term Care, Assisted Living and Retirement Homes within the city of Thunder Bay


Salary Range: $65.210 to 70.750
