Key Responsibilities:
Conduct thorough patient assessments, including medical history reviews, physical exams, and ordering/interpretation of diagnostic tests
Develop and manage individualized care plans based on clinical evaluations and patient needs
Perform clinical procedures such as wound care
Educate patients and their families on disease prevention, health promotion, and the management of chronic conditions
Coordinate care with physicians, specialists, and other members of the care team to ensure holistic and effective treatment
Maintain accurate, timely medical records and documentation
Participate in case management to ensure continuity of care and optimal health outcomes
Qualifications:
Active Nurse Practitioner license (required)
Experience as an NP preferred, but new graduates are welcome to apply
Solid understanding of case management principles and patient care planning
Strong clinical and diagnostic skills with a commitment to patient-focused care
Excellent communication and documentation abilities
