Position Title: Nurse Practitioner (NP) Location: Caritas - Multiple Locations Employment Type: Full-time Reports to: Executive Director
Position Summary
Caritas is seeking a dedicated and experienced Nurse Practitioner (NP) to enhance the medical and mental health services across our therapeutic Bed-Based care programs. The NP will play a pivotal role in integrating medical care into a multidisciplinary team, supporting clients recovering from substance use, co-occurring mental health disorders, trauma, and social marginalization.
The NP will work in close collaboration with Caritas' team, community physicians, and Indigenous and community health partners to ensure safe, consistent, and compassionate care for clients, especially those with complex and chronic needs.
Key Responsibilities
Clinical Care & Coordination
o
Provide holistic, trauma-informed primary and mental health care to clients in Bed-Based care settings.
o
Conduct comprehensive health assessments, order and interpret diagnostic tests, diagnose illnesses, and initiate treatment.
o
Prescribe and manage medications, including controlled substances, opioid agonist therapy (e.g., Suboxone, Methadone), and psychiatric medications.
o
Monitor and respond to medication compliance, side effects, and effectiveness in partnership with clients.
o
Address urgent and emerging health concerns, including infections, injuries, withdrawal symptoms, and mental health crises.
o
Support harm reduction strategies, including overdose prevention, safe supply practices, and sexual health interventions.
Interdisciplinary Collaboration
o
Participate actively in case conferences, care planning meetings, and interdisciplinary meetings.
o
Provide clinical support and consultation to case managers, therapists, and front-line staff.
o
Liaise with walk-in clinics, emergency departments, community pharmacies, and specialists to ensure continuity of care.
Health Education & System Navigation
o
Educate clients on chronic disease management, medication adherence, lifestyle modifications, and wellness practices.
o
Facilitate access to external health and social services including diagnostics, dental care, infectious disease treatment, and reproductive care.
o
Support culturally appropriate care in partnership with Indigenous Elders, Knowledge Keepers, and community organizations.
Documentation & Quality Assurance
o
Maintain accurate, up-to-date clinical documentation and medication records.
o
Ensure compliance with regulatory standards, privacy legislation, and organizational policies.
o
Contribute to quality improvement initiatives and the evaluation of health outcomes.
Qualifications
Required:
o
Registered and in good standing as a Nurse Practitioner (NP) with the College of Nurses of Ontario (CNO)
o
Certification in Primary Health Care (PHC-NP) or Mental Health (PMHNP)
o
Minimum of 3 years clinical experience in addiction, community health, or mental health care
o
Authorized to prescribe controlled substances under the Narcotic Control Regulations
o
Experience working with individuals experiencing homelessness, substance use, trauma, and mental illness
o
Proficiency in trauma-informed, harm reduction, and culturally safe care approaches
o
Strong interpersonal and communication skills, with the ability to build trust and rapport with vulnerable populations
Assets:
o
Experience working in Addiction residential settings or with Community Health Centers
o
Familiarity with the Ontario Health Teams (OHTs) and local health system navigation
o
Experience in a residential, outreach, or mobile care model
o
Bilingual or multilingual capacity is an asset
