As a result of the COVID-19 pandemic and the need to strengthen and promote health protection to patients, staff and members of the public attending in the hospital, prior to the first day of employment with the Hospital, all successful candidates are required to provide proof of COVID-19 vaccination (full vaccination is required) from the Ontario Ministry of Health website or other authorized source. An individual is fully vaccinated if they have provided proof of vaccination in the form of a receipt from COVaxON (the Ministry's provincial system for COVID-19 vaccination information) and 14 days have elapsed from the final dose. In the event that you are unable to be vaccinated as a result of a ground protected under the Human Rights Code, you may submit a written explanation of the ground and any supporting documentation to determine if you are exempt from this requirement.

Position Summary

Nurse Practitioner (NP) Geriatrics Supportive Care Outreach
The Nurse Practitioner (NP) is a registered nurse in the extended class (EC), who demonstrates the competencies to autonomously diagnose order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within their legislated scope of practice. The NP demonstrates the following competencies established by the Canadian Nurses Association Canadian Nurse Practitioner Core Competency Framework (May 2010): Clinical Practice, Collaboration, Consultation and Referral, Research, and Leadership.

Your goal in this position is to perform comprehensive assessment and consultation in palliative care to patients/families with complex and challenging health care needs across all clinical programs and settings. You will work in partnership with program leadership and frontline staff to develop, implement and evaluate best practices in palliative care across the organization. You will demonstrate leadership for promoting awareness and will enhance the understanding of palliative care through the initiation and participation in professional and organizational activities that will advance nursing practice and facilitate improved patient care outcomes in the area of palliative care.
Within this role, you will provide specialized advanced nursing and Supportive Care across various healthcare settings including inpatient, outpatient and outreach, within an inter-professional team model. In addition, you will work collaboratively with the most responsible physician and multidisciplinary team to assist patients and their families manage and plan for illness progression. This position supports the hospital's academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families.

On a practical level, you will
  • Perform comprehensive nursing/medical history, physical and psychosocial assessment of the patient's status, and develop and implement a plan of care to address identified needs of the patient with end-stage cardiac, respiratory and neurodegenerative illness.
  • Provide longitudinal care for patients with chronic progressive, life-limiting illnesses in the community.
  • Provide direct, specialized supportive care and/or consultation to patients, families, health care professionals, and community partners in situations where advanced nursing expertise is required (e.g., complex patient situations with unpredictable outcomes).
  • Collaborate with the interdisciplinary team across the care continuum and healthcare sectors in identifying goals of care and advance end of life care planning.
  • Provide leadership in establishing and meeting goals related to nursing and clinical practice and will align goals with the strategic direction of the program and the organization.
  • Initiate and perform diagnostic and therapeutic procedures for identification and management of symptoms as authorized through medical directives.
  • Educate health care team members in best practice standards as it relates to management of patients with end stage chronic disease.
  • Develop, implement, and evaluate effectiveness of clinical service programs in collaboration with the Program Director and Director of Professional Practice.
  • Perform quality of life evaluations and responses to treatment, ensure care plans are appropriate to the stage of illness, coordinate linkages to community supports/resources, document clinical encounters and communicate effectively with both the Hospital and community providers.
  • Work in collaboration with community providers to ensure home-based services are in place to meet the needs of individuals and families facing life-threatening and/or serious illness.
  • Participate in the tracking of information for the population served into a database, in order to demonstrate effectiveness of the clinical model, as required.
  • Identify, integrate and evaluate research-based best practices and innovations for improving patient care outcomes.
Qualifications
  • Completion of an Acute Care Nurse Practitioner Program (or equivalent) required
  • Registration with the College of Nurses of Ontario (CNO), within the RN (Extended Class) and is a member in good standing required
  • Master in Nursing, with a clinical focus in cardiology, respirology, geriatrics, and/ or palliative care required
  • Membership in Registered Nurses Association of Ontario, preferred
  • Membership in the Nurse Practitioner Association of Ontario, preferred
  • CNA certification in Geriatrics, Cardiology or Hospice Palliative Care or Oncology, an asset
  • Previous experiences as a Clinical Nurse Specialist working within Palliative Care and/or Acute Care Nurse Practitioner, preferred
  • Minimum of three (3) years' recent experience in chronic disease (CHF, COPD, Parkinson's, and/or dementia)/ oncology/ palliative care nursing including experience in symptom management
  • Minimum of three (3) years' clinical nursing experience in palliative care or chronic diseases management
  • Excellent clinical skills in the assessment and management of pain and symptoms as well as end of life care and management of patients with chronic diseases such as heart failure/ COPD/ Dementia
  • Excellent communication and interpersonal skills
  • Demonstrated leadership, commitment, and ability to work collaboratively within an inter-professional practice environment
  • Ability to work autonomously and is motivated to creatively find solutions to patient care and systems issues
  • Excellent dialogue and facilitation, change management and project management skills
  • Knowledge or experience in community/ in-home nursing, preferred
  • Eligibility for cross appointment with Faculty of Nursing, University of Toronto or another academic institution, preferred
  • Valid driver's license and use of a personal vehicle is required
  • Bilingualism in French/English an asset
