As a result of the COVID-19 pandemic and the need to strengthen and promote health protection to patients, staff and members of the public attending in the hospital, prior to the first day of employment with the Hospital, all successful candidates are required to provide proof of COVID-19 vaccination (full vaccination is required) from the Ontario Ministry of Health website or other authorized source. An individual is fully vaccinated if they have provided proof of vaccination in the form of a receipt from COVaxON (the Ministry's provincial system for COVID-19 vaccination information) and 14 days have elapsed from the final dose. In the event that you are unable to be vaccinated as a result of a ground protected under the Human Rights Code, you may submit a written explanation of the ground and any supporting documentation to determine if you are exempt from this requirement.

Position Summary

Nurse Practitioner (NP), Alternative Level of Care Unit:
The Nurse Practitioner (NP) is a registered nurse in the extended class (EC), who demonstrates the competencies to autonomously diagnose order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within their legislated scope of practice. The NP demonstrates the following competencies established by the Canadian Nurses Association Canadian Nurse Practitioner Core Competency Framework (May 2010): Clinical Practice, Collaboration, Consultation and Referral, Research, and Leadership.#LI-BR1

Driven by a fundamental passion and commitment to Patient/Family Centered Care and Inter-professional Practice, you will provide clinical leadership in the assessment of patients and the development, implementation and evaluation of individualized care plans based on advanced practice within the clinical specialty. You will function in an advanced practice role in managing complex acute/chronic illness and promoting wellness. You will also develop strong internal and external networks of consulting partners to ensure the needs of patients are met in collaboration with primary care practitioners and specialists in the community. You are great relationship builder, and foster close contacts with peers across the entire organization. This position supports the hospital's academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. You will also take on the shared responsibility for ensuring that health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families.

On a practical level, you will
  • Perform comprehensive nursing/medical history, physical and psychosocial assessment of the patient's status, and development and implementation of a plan of care to address identified needs of patients
  • Provide direct, specialized supportive care and/or consultation to patients, families, health care professionals, and community partners in situations where advanced nursing expertise is required (e.g., complex patient situations with unpredictable outcomes)
  • Provide leadership in establishing and meeting goals related to nursing and clinical practice; accountable to align goals with the strategic direction of the program, the organization and the Regional Geriatric Program of Ontario
  • Initiate and perform diagnostic and therapeutic procedures for identification and management of medical issues as authorized through the use of medical directives
  • Educate health care team members in best practice standards as it relates to the management of complex and frail geriatric patients
  • Develop, implement, and evaluate the effectiveness of clinical service programs in collaboration with the Program Director and Director of Professional Practice, Research and Education and Clinical Team Manager
  • Engage in or conduct nursing research and participates in inter-professional research projects, as required
  • Engage in scholarly activities, such as presentations and publications, and initiate Community of Practice for Nurse Practitioners supporting ALC Units.
  • Contribute to the ongoing maintenance of resource management/utilization in accordance with priorities of the Medical/Eldercare Program and Regional Geriatric Program of Ontario
Qualifications
  • Must obtain and provide to the Hospital a current Police Vulnerable Sector Check
  • Masters in Nursing, or in a health related field (e.g. MH Sc), and completion of a Nurse Practitioner Program
  • Current registration from the College of Nurses of Ontario as an RN (EC)
  • Current completion of a 'Prescribing Narcotics and Controlled Substances' course approved by the CNO
  • CNA certification in Enterostomal Therapy (CAETN (c)) an asset
  • Membership in the Registered Nurses Association of Ontario and/or Nurse Practitioner Association
  • Minimum of 3 - 5 years clinical nursing experience in medicine and/or geriatrics
  • CNA certification in Gerontology Nursing an asset
  • Experience in nursing education, research, program planning and evaluation methodologies
  • Knowledge of Regulated Health Professions Act (RHPA) as it relates to Legislated scope of inter-professional practices
  • Proficient with computers, MS Word, internet searches, data entry and patient information systems
