1. Summary

The Harbour Landing Village Nurse Practitioner is part of the multidisciplinary Care Team, providing comprehensive medical support to our Personal Care Home (up to 36 residents), Community Living Group Home (5 residents), and Independent Living (up to 54 residents). This role will focus on the overall health and well-being of our residents ensuring they receive high-quality, personalized care while following the Pillars for Successful Aging which encourages our residents to live life to the fullest by emphasizing productive engagement, social connection, physical activity, and proper nutrition in a welcoming and caring environment. This position will practice within the framework of evidence-based primary care, holism, health promotion, and disease and injury prevention.

The HLV Nurse Practitioner's skills and knowledge gained through additional education and experience will allow to utilize their in-depth knowledge of resident care with the legal authority and autonomy to order diagnostic tests, perform specific procedures in the management and treatment of common acute and stable conditions as well as prescribe pharmaceuticals as required. The goal of this position will be to build partnerships with residents, families and the community as a whole. Clinical assessments and treatment strategies will reflect an in-depth knowledge of the geriatric care, disability supports and holistic management of the entire person.

2. Responsibilities

Resident Health Examinations & Baseline Assessments

: Support initial examinations for residents to establish a baseline of health and identify any immediate medical needs.

Quarterly Health Monitoring & Assessments

: Perform quarterly health assessments to monitor residents' ongoing health status and adjust care plans as necessary.

Medical Care Management & Oversight

: Manage and oversee ongoing medical care and treatment for residents, ensuring adherence to best practices and individualized care plans.

Illness Diagnosis & Treatment Collaboration

: Diagnose and treat illnesses, collaborating closely with the care team to ensure optimal health outcomes.

Diagnostic Testing & Treatment Decisions

: Order and interpret diagnostic tests to inform treatment decisions and provide timely interventions.

Medication Management & Prescriptions

: Prescribe medications, managing residents' pharmacological needs while considering potential interactions and side effects.

Chronic Disease Management & Care Plans

: Manage chronic diseases such as diabetes and COPD, through comprehensive care plans that promote self-management and quality of life.

Mental Health Support & Holistic Care

: Address mental health concerns by providing supportive care, referrals, and interventions as needed, integrating a holistic approach to health.

3. Skills, Qualification, & Requirements

:

Registered Nurse with advanced education and clinical training licensed and certified as a Nurse Practitioner in Saskatchewan 3 + years of general care or long-term care experience - geriatric experience an asset. Possesses a strong foundation in addressing the physical, mental, emotional, and spiritual health needs of our residents. Familiarity with the Ministry of Health Personal Care Home Licensing requirements and regulations is an asset. A valid basic life support CPR C & Standard First Aid. Strong communication skills, written and verbal. High level of organization skills with strong attention to detail. Enjoys working as part of a team and can lead a team efficiently. Strong administration skills for reporting and documentation. Able to have hard conversations with good conflict resolution skills. Maintains professional, supportive and responsive interpersonal communication skills.
