Chancellor Park has an exciting opportunity for a motivated, passionate, and energetic Nurse Practitioner who wants to work within a dynamic interdisciplinary team who are committed to providing the highest standard of care for its residents while creating a positive work environment for its staff.
Reporting to the Administrator, the Nurse Practitioner is a primary care provider to residents and works within their legislative scope of practice as described and outlined by the College of Registered Nurses of Newfoundland and Labrador. The Nurse Practitioner ensures collaboration with the Resident, the family/care giver, and the health care team in the development, implementation and evaluation of the Resident's plan of care. They provide leadership and mentorship to the nursing staff that enhances their knowledge, assessment skills, and ability to care for Residents; while also leading and collaborating in research, education, and evidence-based practice initiatives to optimize the resident, organization and health system outcomes.
Duties and Responsibilities:
Direct Care to Residents
Provides comprehensive primary care to residents as part of the interdisciplinary health care team.
Performs a person-centered health assessment of residents
Provides person-focused health education.
Participates in regular care conferences.
Manages the care of residents by providing pharmacological, complementary and/or counselling interventions, and performs procedures within the NP scope of practice.
Orders and/or performs appropriate screening and diagnostic investigations, interpreting results and assuming responsibility for follow-up.
Provides outreach and transitional services to residents who return to the community.
Liaises with discharge planning services for hospitalized residents to ensure a smooth transition back to the Facility.
Assists, supports, guides and provides consultation to Executive Director, nursing staff and interdisciplinary team members regarding challenging clinical situations.
Makes referrals to specialized consultants, services and other health providers.
Advocates for and provides palliative and end-of-life care.
Research, Education, and Leadership
Participates in creating an organizational environment that supports the safety quality of resident care and life, collaborative practice, and professional growth in clinical staff by integrating best practices in resident care.
Participates in internal (e.g. Professional Advisory Committees, Quality Committee) and external committees.
Provides leadership in quality improvement initiatives and in developing and implementing strategies to optimize the integration of illness and injury prevention, health promotion, health maintenance, rehabilitation and restorative care activities.
Provides formal and informal teaching and coaching in the management of clinical care to interdisciplinary team members, serving as resource person, educator and role model.
Contributes to planning, implementing and evaluating learning resources and health education programs for residents, families and substitute decision makers.
Represents the organization's mission, vision and values in all relationships with internal and external stakeholders
Qualifications and Experience:
Current Nurse Practitioner License and registration with the College of Registered Nurses of Newfoundland and Labrador.
Experience and Education/Training in Long Term Care and Gerontology would be a strong asset.
Post-graduate certificate and experience in palliative care an asset.
Current CPR certification designed for health-care professionals and standard first-aid
Excellent interpersonal and communication skills
Proficiency in Microsoft Office programs (Word/Excel/Publisher/Power Point)
Knowledge of the requirements of the Provincial LTC Regulations.
