Job Description

The Scarborough Academic Family Health Team (SAFHT) is governed by a Mixed Board (Physicians + Community Members). It is a multi-site Family Health Team located within Eastern Toronto using a collaborative, inter-professional model to provide comprehensive primary care for patients. SAFHT is a non-profit organization that receives funding from Ontario Health and provides primary health services to over 31,000 patients in the Scarborough Community.

ACCOUNTABILITY



The Nurse Practitioner (NP) reports to the Executive Director of the Scarborough Academic Family Health Team (SAFHT) and as a primary care provider operates in accordance with the policies of the SAFHT. The NP is to practice within the scope and according to the standards of practice for Nurse Practitioners as outlined in the following documents:

College of Nurses of Ontario's "Practice Standards" (www.cno.org)

Nursing Act (1991) and Regulated Health Professions Act (1991)

RESPONSIBILITIES



Provides primary care functions, consulting and supporting services for patients referred by team family physicians, unaffiliated physicians and other team clinicians. Works autonomously and collaboratively in providing individual patient care assessment and treatment in core clinical programs, including complex and chronic disease management, mental health, cancer prevention and palliative care. Provides services to patients with a focus on health promotion, patient education, and disease prevention. Acts as a strong resource to all the Interdisciplinary Health Providers (IHPs) and family physicians within SAFHT. As one of SAFHT's two full time NPs, the NP will share the clinical lead for the Chronic Disease Management Program Teams and the Complex, Vulnerable (FOCUS) Program Provide teaching and resources to the RNs regarding Chronic Disease Management programs, care for seniors and shared palliative care. Work closely with the Executive Director and other staff to build strategic approaches to programs from delivery through to evaluation. Participates in the development, implementation, monitoring and evaluation of SAFHT's programs and services for individuals and families of the Scarborough community Sees patients in the community through home visits Collaborates with physicians and other allied health professionals to develop and discuss individualized care plans with the patient and family based on best practices Facilitates appropriate internal and external education sessions

EDUCATION AND CERTIFICATION



Registered Nurse in the extended class with a certificate issued by the College of Nurses of Ontario. Nurse Practitioner certificate in Primary Care. Baccalaureate of Science in Nursing is required, Master's degree preferred. Current BCLS certification. Member of RNAO required, NPAO preferred, and evidence of liability insurance.

QUALIFICATIONS



Experience in a family health team and/or community setting is preferred. Experience in working with multiple care teams in diverse settings is valued. Being able to role model, lead and mentor colleagues and students Experience in the areas of chronic disease management, seniors care, mental health and the provision of cancer care screening and palliative care is preferred. Independent health care assessment and patient management skills are required. Experience working with inter-professional health care teams and providing full scope collaborative support within that setting. Demonstrated experience in ethno-culturally diverse environments is preferred. Experience working with vulnerable populations Demonstrated ability to prioritize and work effectively in a fast-paced environment. Demonstrated ability to contribute in program development, implementation and evaluation is preferred. Demonstrated commitment to continuous learning and innovation. Strong written, verbal interpersonal and communication skills are required. A second language would be an asset. BLS certification required. Computer skills are required (TELUS- Electronic Medical Record, Microsoft Office). A satisfactory and current Criminal Background and Vulnerable Sector Screening and proof of immunization required. A valid Ontario Driver's License, valid insurance, and own vehicle required.
