The North Lambton Community Health Centre is currently seeking applicants for the following position:
1.0 FTE NURSE PRACTITIONER
37.5 hours/week, fulltime with benefits
The Nurse Practitioner (RN-EC) delivers comprehensive primary care and contributes to the planning, implementation, monitoring, and evaluation of programs and services for individuals, families, and the community. As a key member of the interdisciplinary team, the Nurse Practitioner works both independently--providing the full scope of primary and outreach care--and collaboratively, by consulting with other clinical team members as appropriate.
Qualifications
:
Undergraduate degree in nursing from a recognized university (or equivalent combination of education and experience).
Registration in the Extended Class with the College of Nurses of Ontario.
Three to five years nursing experience in a community setting or combination of community and hospital or public health settings; experience in a primary care setting, an asset.
Experience in program development, implementation, monitoring, and evaluation.
Proficiency in the use of computers and various software applications.
Participates in CNO Quality Assurance Program.
Has and maintains CNO Professional Liability Insurance.
This is a fulltime position, working 37.5 hours per week, Monday to Friday. This position will be based at our West Lambton Site, with allocated time at outreach sites. Experience will be taken into consideration on our exciting new 6-step wage grid. Vacation benefits, medical benefits, the Healthcare of Ontario Pension Plan and paid vacation are included.
