Le Centre de sante communautaire Chigamik Community Health Centre est un organisme sans but lucratif qui s'engage a fournir des soins de sante primaires bilingues et adaptes a la culture, la prevention des maladies et la promotion de la sante. Nous avons pour mission d'offrir des programmes et des services holistiques adaptes a la culture pour doter nos collectivites des outils necessaires a atteindre la sante et le mieux-etre optimaux grace a la sensibilisation, a la promotion de la sante et a la prevention des maladies.
Apercu du poste :
Relevant de la direction des services cliniques et paramedicaux, l'IP s'occupe de la prestation des services de soins primaires aux personnes et aux familles en mettant l'accent sur une approche holistique a la prevention, a la promotion et a la gestion de la sante aupres des collectivites francophones en particulier. L'IP en soins primaires a une liste de patients inscrits a sa charge et travaille en concertation dans le cadre d'une equipe de soins primaires interprofessionnelle.
Qualifications requises :
La maitrise des deux langues officielles est une exigence de ce poste.
Maitrise en soins infirmiers ou l'equivalent obtenue d'une universite reconnue.
Actuellement inscrite/inscrit a l'Ordre des infirmieres et infirmiers de l'Ontario dans la categorie superieure; (infirmiere praticienne/infirmier praticien).
La preference est accordee a la candidate/au candidat qui possede au moins trois ans d'experience a oeuvrer en tant qu'infirmiere praticienne/infirmier en soins primaires.
Excellentes competences en evaluation, diagnostic, traitement et education en matiere de sante.
Connaissance des personnes et des communautes Premiere nation, Metis et Inuit et/ou francophone et experience a oeuvrer aupres d'elles.

NURSE PRACTITIONER (RN-EC)

Our mission is to provide culturally relevant holistic programs and services to equip our communities to achieve optimal health and well-being through awareness, health promotion, and illness prevention.
Job overview:
Reporting to the Director, Clinical and Allied Health Services, the NP will focus on providing Primary Care services to individuals and families, emphasizing a holistic approach to health prevention, promotion and management, with a focus on our Francophone communities. The Primary Care NP will carry a roster of patients and will work collaboratively as part of an Interprofessional Primary Care Team.
Required qualifications include:
Fluency in both official languages is a requirement for this position.
Master's Degree in Nursing or equivalent from an accredited university
Currently registered with the College of Nurses of Ontario in the Extended Class; (Nurse Practitioner)
The preferred candidate will have a minimum of three (3) years' experience as a Nurse Practitioner working in Primary Care.
Excellent assessment, diagnosis, treatment and health education skills;
Knowledge of/experience working with First Nations, Metis, and Inuit and/or Francophone individuals and communities is preferred.
