Currently looking for a Nurse Practitioner to provides specialized care and management of clients living with chronic disease and multiple co-morbidities. The NP team consists of 3 NP\'s working collaboratively and supporting each other. Having additional education and training to provide advanced clinical skills, make critical management decisions, and facilities consistent client-centered care as a member of a multi-disciplinary team. The Nurse Practitioner collaborates with Home Care staff, attending physicians and specialists to acquire and utilize best practice, evidence and resources needed for individual situations and to adjust and optimize program delivery. The successful incumbent would participate in program committees, quality improvement programs, and may lead nursing research initiatives as well as providing education to provide mentorship to new NPs, NP students and other healthcare providers. To excel in this role, you will need good communication skills and the ability to maintain effective interpersonal relationships. You must be able to work in a collaborative care setting. As the successful candidate, you will demonstrate ongoing responsibility for maintaining your own continuing competence through professional development activities.
