Position: Nurse Practitioner -Primary Healthcare
Organizational Background:
Portsmouth Medical Clinic is seeking a Nurse Practitioner to join our primary care team. The successful applicant will provide comprehensive primary care services within the full NP scope of practice. These services will include core primary care, health promotion, disease prevention and chronic disease management as supported by the organization.
TThe clinic also provides pre-employment communicable disease screenings.
Position Summary:
The NP is an integral and valued member of the Portsmouth Medical Clinic. The NP provides care of patients and families following the guiding principles and standards of the College of Nurses of Ontario. The NP advocates for quality of life for the patients by modelling leadership, ethics, advocacy, and excellence in the assessment process ensuring patient-centred care. The NP actively leads and contributes to team meetings and care planning.
Overview of Duties:
Completes independent system-focused assessments to facilitate diagnosis and follow up
Collaborates and assists the physician with assessments and procedures
Completes telemedicine and virtual assessment as needed
Liaises with the team, including other health care providers, patients, and family, to provide and promote excellence in care and the patient experience
Collaborates with community partners to support best-practice guidelines
Provides leadership and direction to the direct care staff
Administers medications and treatments as prescribed within the RN scope of practice
Documents all care, assessments, and observations as per college guidelines
Mentors, supports, and encourages newly hired staff and students

Qualifications:
Current unrestricted NP Certificate of Competence from the College of Nurses of Ontario
Current Professional Liability Protection per CNO requirements
Reprocessing in the Community Setting certificate from Public Health Ontario considered an asset
Excellent leadership, organization, critical-thinking, problem-solving, and computer skills
Fluent in both written and spoken English

Hours:
Monday through Thursday, 7:30am until 4pm, Friday 7:30am until 1pm. One evening clinic twice per month and one 3 hour weekend shift every three months

Full-time, Term six months with possibility of renewal
