to join our multidisciplinary healthcare team. The ideal candidate will provide comprehensive, person-centered care to patients of all ages -- with a strong focus on
mental health support, neurodivergent care (including ADHD and autism), and the management of anxiety, depression, and chronic conditions

Key Responsibilities
Conduct thorough, holistic assessments addressing patients' mental and emotional health.
Diagnose and manage conditions such as
ADHD, anxiety, depression, and related behavioral health concerns
.
Develop and implement individualized care plans that integrate medical, behavioral, and psychosocial needs.
Provide preventive care, patient education, and lifestyle counseling tailored to each patient's goals.
Manage chronic diseases (e.g., diabetes, hypertension) while addressing emotional and behavioral health factors that impact wellbeing.
Collaborate closely with physicians, therapists, nurses, and case managers to ensure coordinated and compassionate care.
Maintain accurate and timely documentation in electronic health record systems such as Epic or Cerner.
Participate in care planning and follow-up for patients needing additional behavioral or social support.
Stay up to date on current best practices in
integrated primary and mental healthcare
.
Qualifications
Master's degree in Nursing and current Nurse Practitioner (NP) license in good standing.
Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology NP preferred.
Experience working with
children, teens, and adults
, particularly those with
ADHD, anxiety, depression, or other neurodivergent conditions
.
Background in
behavioral health integration
or trauma-informed care is highly desirable.
Strong communication and interpersonal skills to foster trust and rapport with patients and families.
Demonstrated ability to work effectively as part of a multidisciplinary team.
Excellent clinical judgment, organization, and documentation skills.
