- Provide primary and acute medical care to seniors in Retirement Homes working at full scope of practice in the Mississauga area.
- Conduct comprehensive assessments, diagnose and treat acute and chronic illnesses
- Conduct person-centered comprehensive health histories
- Collaborate with interdisciplinary healthcare teams to develop and implement patient care plans
- Collaborate with the resident nursing staff and their family, to develop a person-centered plan of care
- Regular medical assessments with patients
- Interpret diagnostic tests, such as specimen collection processing and medical imaging
- Participate in family care conferences
- Educate staff, patients and their families on disease prevention, management, and treatment options
- Provide specialized care for patients with , dementia, and other complex medical conditions
- Early identification of illness allowing timely intervention and reduce hospital transfers
- Resident Support & Education - age specific health promotion, education, disease and injury prevention support
- Palliative Care Management
Qualifications:
- Current valid CNO registration as an RN (EC)
-BScN required, - Master's degree in Nursing from an accredited program
- Relevant clinical and health assessment experience as a licensed Nurse
- Current CPR certification
- Certified Diabetes Educator (CDE) certification an asset
- Certification as a Nurse Practitioner in the appropriate specialty area (e.g., Family, Adult or Gerontology)
- Experience working with elderly patients with memory care needs or dementia is preferred
- Strong clinical skills and knowledge of evidence-based practice guidelines
- Excellent communication and interpersonal skills to effectively collaborate with patients, families, and healthcare teams
- Ability to prioritize tasks, manage time efficiently, and work independently
