Nurse Practitioner - Primary Geriatrics and/or Palliative Care (Retirement Homes)



Position Type: Independent Contractor

Location: Three Locations in Toronto (Grenadier, Leaside, and Kingsway Retirement Home)

Start Date: Immediate / January 2026

Hours: 10-20 hours per week

Reporting to: Dr. Mateen Akhtar, M.D.

Compensation: $80 per hour

Overview:



The Nurse Practitioner (NP), working in close collaboration with Dr. Akhtar, will lead the delivery of high-quality primary care to residents of

Grenadier, Leaside

and

Kingsway Retirement Homes

. This role is focused on

geriatric and palliative care

, supporting older adults with complex health needs through chronic disease management, preventive health, and thoughtful, personalized care planning. The NP will be responsible for conducting comprehensive assessments, initiating evidence-based treatment plans, and leading discussions around

goals of care

,

advance directives

, and

power of attorney (POA)

Responsibilities:



1.

Geriatric Primary Care:

Deliver comprehensive, patient-centered care tailored to the needs of aging residents, managing both acute issues and chronic conditions within the NP scope of practice.

2.

Health Assessments:

Perform full clinical evaluations, including histories and physical exams, to develop clear, actionable care plans.

3.

Palliative Care:

Provide symptom management, end-of-life planning, and ongoing support for residents receiving palliative or comfort-focused care.

4.

Goals of Care & POA Discussions:

Facilitate meaningful conversations with residents and families regarding goals of care, code status, and POA documentation, aligning medical interventions with patient values.

5.

Collaboration:

Work closely with physicians, RNs, RPNs, and allied health providers while maintaining clinical autonomy and consulting as needed.

6.

Consultation & Referrals:

Coordinate specialist referrals, diagnostics, and community supports to ensure seamless transitions and continuity of care.

7.

Documentation:

Maintain timely and accurate paper-based medical records, in accordance with legal, regulatory, and professional standards.

8.

Professional Development:

Engage in continuous learning and stay informed on evolving best practices in geriatrics, palliative medicine, and primary care.

9.

Compliance:

Follow all clinical and organizational policies as set by the retirement homes and regulatory bodies.

10.

Travel (Optional):

Opportunities may arise to support additional retirement homes across the GTA.

Requirements:



1.

Licensure:

Registered Nurse in the Extended Class (EC) with the College of Nurses of Ontario with active OHIP billing number.

2.

Education:

Completion of an accredited Primary Health Care Nurse Practitioner program.

3.

Clinical Expertise:

Strong diagnostic and therapeutic skills with experience in managing complex geriatric patients.

4.

Geriatric & Palliative Focus:

Demonstrated interest or experience in elder care, end-of-life care, and advance care planning.

5.

Teamwork:

Effective collaboration within interdisciplinary teams to ensure coordinated, holistic care.

6.

Professionalism:

Commitment to excellence, accountability, and continuous improvement.

7.

Documentation:

Comfortable managing paper charts, with basic proficiency in electronic systems where applicable.

8.

Local Knowledge:

Familiarity with the health needs and community resources of the GTA is an asset.

9.

Health Records:

Accuracy and clarity in paper-based charting; organized, complete, and legally compliant.

10.

Communication:

Strong interpersonal and communication skills to navigate sensitive conversations with residents and substitute decision-makers.

11.

Cultural Competence:

Culturally safe care for diverse and marginalized populations.

12.

Primary Care Experience:

Preferred experience in long-term care, community health, or palliative settings, with the ability to work independently
