The Opportunity



We are looking for

Nurse Practitioner

to join our Prenatal and Family Health Clinic team at CUPS. This opportunity involves working 24 hours per week, spread over three weekdays (Monday through Friday). Specific days will be determined upon hire.

CUPS

Nurse Practitioner

is responsible for providing trauma-informed, low barrier, primary care services to vulnerable Calgarians with complex health and social needs. As a CUPS Nurse Practitioner you will be part of CUPS Prenatal and Family Health Clinic team, providing holistic care to women and gender diverse individuals with uteruses. You will facilitate the provision of health care to people through health assessment, health education and health promotion. You are provided the opportunity to work with individuals, families and the community to achieve client-defined goals and enhance wellness and quality of life.

You are part of a Health Team who provides services within CUPS primary service delivery models of integrated and trauma-informed care by operating in partnership with internal CUPS services, linking patients to appropriate supports both within CUPS and externally as needed.

STARTING

HOURLY SALARY RATES:

$57.69 to $61.69

Key Responsibilities:



Work interdependently with the other health care team members to provide nursing services in an extended practice context and provide advanced, comprehensive assessment, diagnosis, and treatment, including completing referrals and ordering diagnostic tests as required Assess social and mental health supports and provide referrals to internal staff and external agencies Assess for disability eligibility, including Income Assistance, AISH, EI Disability, CPP Disability, and private or 3rd party assessments, along with completing the required forms and documentation Provide evidence-based pediatric healthcare, including developmental screens and providing anticipatory guidance Facilitate complex case management for clients, families, and communities Support wellness in chronic disease management Participate in formal and informal health teaching, including the preventative health concepts concerning health issues Facilitate the design and implementation of health education programs with a focus on prenatal education Assist in developing programs through partnerships with the community, based on needs expressed by health care providers and the community Support research activities, including the composition and recruitment of subjects, data collection, and evaluation Organize continuing education for clinic staff and volunteers, as well as clinical outreach opportunities to provide improved access to prenatal and family Health Services Complete all CUPS mandatory training to ensure your practice and interactions are congruent with agency expectations and legislated parameters Facilitate the maintenance of accurate and complete health records that meet the demands of interdisciplinary care, the Alberta Health Information Act, and all other relevant provincial and federal legislation.

Create an approachable, caring and trauma-informed environment for clients in each interaction Provide holistic primary care services in alignment with the scope of practice and in accordance with the Province of Alberta's health professions Act and Professional Standards under the College and Association of Registered Nurses of Alberta Promote, and model CUPS team-based care approach and ensure the integrity of CUPS client care philosophy is maintained Participate in the CUPS mission to support the learning and development of new professionals through preceptorship activities and support the education and training of all learners stationed within CUPS Collaborate day-to-day with a multidisciplinary team to provide medical and social support in a primary care setting Demonstrate a commitment to best practice and evidence-based medical care through participation in quality improvement work Participate in daily huddles with a multidisciplinary team to support closed-loop communication and timely delivery of services Work collaboratively by creating rapport with team members within the department and across the organization Act as an ambassador of CUPS' mission and values both internally and externally

Stakeholder Relations:



Develop and maintain excellent communication with Prenatal and Family Health interdisciplinary team, incorporating directives, feedback and procedure updates into work practice to ensure activities are congruent with the needs of integrated care Foster a healthy and safe working environment per CUPS health & safety training, policies and procedures Participate in CUPS in organizational activities such as in-house training, team activities and lunch n' learns as schedule allows to maintain strong connections and awareness of organizational developments Work collaboratively with staff throughout the agency to ensure an integrated and coordinated response to care for clients, as well as external agencies/partners as required to support the care of clients

Required Qualifications:



Minimum Education of a Master's of Nursing via an approved Nurse Practitioner Program Active or eligible licensure with the College of Registered Nurses of Alberta CRNA as a Nurse Practitioner in good standing Understands and is aligned with philosophies around trauma-informed practice, Adverse Childhood Experiences, the socio-environmental determinants of health, and issues related to poverty, mental health and substance use Demonstrated understanding and a commitment to an integrated and team base model Is knowledgeable in harm reduction practices and the social determinants of health Experience in providing opioid agonist treatment is an asset Experience in community health & public health sciences is an asset Knowledge of community resources connected to the provision of health promotion and illness prevention-related services Demonstrated experience in the development of proposals and evaluating programs an asset An engaged team player capable of interacting professionally with multi-disciplinary health care staff at CUPS with a collaborative attitude Excellent written, communication, organizational and time management skills with the ability to take the initiative and work with limited supervision but within the guidelines of the position Ability to work efficiently, both independently and as part of a multi-disciplinary team Computer competency & experience with an electronic medical record is necessary
