The Nurse Practitioner (NP), having a specialty-based focus, functions in an expanded nursing role demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and promote wellness.
The successful candidate will work with Pediatric Cancer Survivors in two main Survivorship Clinics, focusing on patients diagnosed with pediatric CNS cancers and those diagnosed with non-CNS cancers in an outpatient setting. The Pediatric Aftercare NP will utilize the Children\'s Oncology Group recommendations to create surveillance recommendations and follow up plans for both survivors and to disseminate with other primary care providers. The Pediatric Aftercare NP will be responsible for organizing surveillance tests, following up on results, initiating referrals to specialist providers. The successful NP candidate should have a basic understanding of fertility and impact from cancer treatment. The NP will work with a multidisciplinary team to provide comprehensive care. The NP will play a leadership role in close collaboration with the radiation oncology physicians working in the Pediatric Aftercare clinic. The NP will receive referrals from other providers to access the Survivorship Clinic and will be responsible for referring patients to other Survivorship Clinics across Canada. This NP role will be a combination of in person and virtual clinics. This role will involve trying to set up surveillance tests closer to survivors\' homes when they live outside of the GTA. This NP role will also involve collaborating with the Sick Kids Aftercare Clinic and attending 4 Transition Clinics a year, and a monthly CNS clinic at the Hospital for Sick Children. Preferably the successful candidate has a FNP NP designation.
