The Nurse Practitioner provides advanced clinical assessment, diagnosis, and treatment for residents within the long-term care home. The NP works collaboratively with the interdisciplinary team to enhance resident outcomes, reduce hospital transfers, and support high-quality, person-centered care. This role is ideal for an NP who values autonomy, relationship-building, and providing comprehensive geriatric-focused care in a supportive LTC environment.

Key Responsibilities
Clinical Assessment & Care
Conduct comprehensive health assessments and develop individualized care plans
Diagnose, treat, and manage acute and chronic conditions within the NP scope
Order and interpret diagnostic tests, labs, and screenings
Prescribe medications and treatments in accordance with legislative authority
Provide ongoing monitoring and follow-up for residents with complex needs
Collaboration & Communication
Work closely with the Medical Director, physicians, Director of Care, RNs/RPNs, and allied health staff
Participate in interdisciplinary care conferences and family meetings
Support advance care planning, goals of care discussions, and palliative approaches
Quality Improvement & Leadership
Identify opportunities to improve care delivery, safety, and outcomes
Support best practices, including falls prevention, skin integrity, medication management, and responsive behaviours
Provide education and mentorship to nursing staff
Support compliance with LTC legislation, policies, and accreditation standards
Resident & Family Support
Build strong relationships with residents and families
Communicate changes in condition, treatment plans, and care strategies with clarity and compassion
Support transition-of-care planning to minimize avoidable hospital transfers

Qualifications
Current registration in good standing as a
Nurse Practitioner (Primary Health Care NP-PHC or Adult NP)
with the College of Nurses of Ontario
Experience in long-term care, geriatrics, primary care, or chronic disease management preferred
Strong clinical assessment skills and experience managing complex resident populations
Excellent communication, problem-solving, and decision-making abilities
Ability to work independently within the NP scope while collaborating effectively with the team
Compassionate, resident-centered approach with sensitivity to the LTC environment
