Reporting to the Manager of the Palliative Care program, the Nurse Practitioner works closely with the interprofessional team, Program Directors and Program Leadership.

Responsibilities:

  • Practice in accordance with the Regulated Health Professions Act and the Nurse Practitioner Practice Standards set by the College of Nurses of Ontario.
  • Conduct comprehensive patient assessments, including physical, psychosocial, and spiritual needs.
  • Work with teams and patients/families to develop individualized care plans for patients with palliative care needs. This may include patients in the emergency department, outpatient areas and provision of home visits.
  • Function as Most Responsible Provider post-discharge from hospital for end of life care or ongoing palliative pain and symptom management.
  • Provide advanced nursing care, including symptom assessment/management and appropriate medication prescribing in hospital, clinic and patients\xe2\x80\xb2 homes.
  • Facilitate and provide leadership in the development of the integrated Scarborough Palliative Care Program
  • Support development of, and participate within the Palliative Rapid Response Team as an integral component of the integrated Scarborough Palliative Care Program.
  • Act as a resource for other healthcare professionals to promote best practices in palliative care.
Requirements:
  • Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP(primary health care) required
  • Master\'s Degree in Nursing/Nurse Practitioner (MN-NP) or Master of Science in Nursing with successful completion of a post-Masters Nurse Practitioner Certificate Program from a recognized school of graduate studies required.
  • Must have completed a CNO-approved controlled substances education
  • Minimum of 3 years of recent clinical experience in palliative care or related field
  • Valid Ontario Driver\'s license and access to a vehicle required
  • Working knowledge of the Canadian Core Competency Nurse Practitioner Framework and Canadian Nurses Association Competency Framework for Advanced Practice Nursing, with ability to fully apply those competencies.
  • Strong clinical assessment and management skills with sound knowledge of clinical therapeutics
  • Excellent communication, collaboration, and leadership skills
  • Current BCLS provider and competency.
  • Evidence of strong mentoring, teaching, organizational and prioritization skills, with the ability to work well in stressful situations
  • Excellent interpersonal, communication, and documentation skills with the ability to work collaboratively with patients, families, all levels of staff and the community
  • Must demonstrate a commitment to customer service when dealing with patients, colleagues, volunteers and other members of the community and public
