Job Description

Reporting to the President & CEO, and under the guidance of the Medical Director, the Nurse Practitioner (NP) plays a crucial role in expanding access to primary health care and promoting evidence-based practice to enhance the quality of care in the Long-Term Care Home. Through proactive care and timely interventions, the NP contributes to a higher quality of life for residents and improves family satisfaction. The NP serves as a liaison between physicians, registered staff and residents/families ensuring clear and timely communication of health care information.

In collaboration with the VP/AVP of Nursing, the NP leads and mentors the nursing team, enhancing staff knowledge and assessment skills that assure overall excellence congruent with the Mission, Vision and Values of the Home and the Person-Directed Care Philosophy of the Home.

Responsibilities of the successful candidate:



Works collaboratively with physicians, Registered Nurses (RNs), Registered Practical Nurses (RPNs), pharmacists, physiotherapists, and social workers to plan, implement, coordinate and evaluate resident care in consultation with residents and their families Conducts comprehensive and focused physical examinations of residents as well as monitors chronic and acute medical conditions Diagnoses common illnesses and manages stable chronic conditions within NP scope of practice Assesses and treats wounds, championing evidence-based wound care practices among nursing staff. Willing to complete NSWOCC certification with Home's support In collaboration with the attending physician:
o Develops and implements treatment plans, prescribes medications, orders diagnostic tests and therapeutic interventions, and provides follow-up care

o Advocates for residents' needs and provides person-directed palliative and end-of-life care, including supporting residents and families in discussions about goals of care and end-of-life wishes

o Assists with smooth transitions of care, including liaising with hospital discharge planners for residents returning from a hospital stay

o Communicates and counsel residents and their families on care plans, health promotion, disease prevention, and treatment interventions

Engages in quality improvement initiatives by integrating current research and evidence-based practices to enhance resident care outcomes. Collaborates with the VP of Human Resources and VP/AVP of Nursing to implement remedial actions and drive continuous improvement across care delivery systems. Engage in ongoing professional development to maintain their clinical competence and stay current with legislative changes and medical advancements Completes required reporting and documentation for regulatory and quality assurance purposes Provides education to residents and their POAs on health maintenance, nutrition, fall prevention, and medication adherence. All other duties as assigned.

EDUCATION OR EQUIVALENT KNOWLEDGE



Bachelor of Science in Nursing (BSN) Graduate of a CNO-approved Nurse Practitioner Program Current Certificate of Competence from the CNO in the Extended Class Master of Nursing Preferred*
Three (3) years clinical experience pertinent to the position Preferred*

QUALIFICATIONS



Demonstrated leadership ability in working within interdisciplinary environment. Possess an understanding of polypharmy, palliative & end-of-life care, mental health & dementia care, fall prevention, wound care and mobility support Competent in electronic health records systems. Computer literacy (Word, Excel, Power Point) and familiarity with web-based systems pertinent to LTC. Ability to multitask and manage multiple deadlines. Demonstrated strong communication skills for working with interdisciplinary teams and families Experience in geriatrics, chronic disease management, or LTC settings is strongly preferred
