GENERAL RESPONSIBILITIES & ESSENTIAL FUNCTIONS:
Direct Patient Care:
Provide in-clinic care alongside local Gastroenterologists, (episodic primary care, preventative care, chronic disease management, patient education, coordination/transition care).
Responsible for taking medical history and patient assessment for in-clinic patients
Support Inflammatory Bowel Disease (IBD) patients pre/post-infusion (assessment, monitoring, symptom management, education, navigation).
Document thoroughly in the Electronic Medical Records; order appropriate tests/consults; collaborate closely with the interprofessional team.
Education and Community of Practice
Develop and deliver patient and team member education (e.g., self-management, meds adherence, infusion readiness).
Help build the IBD community of practice (case rounds, journal clubs, guideline adoption, outcomes review).
Assist with referrals to other healthcare providers
Help build out other areas of excellence including Dermatology, Rheumatology and Neurology
Clinical Operations and Quality (Biologic Infusion Clinics)
Support day-to-day operations and nursing workflows; mentor RNs; promote best practices for safe biologic infusions.
Draft/maintain policies, procedures, and clinical pathways; lead quality & safety initiatives (audits, incident reviews, KPI tracking).
Contribute to business development (service design, new clinic offerings, partnerships).
QUALIFICATIONS
Experience in a leadership role with responsibility for operations and business development.
4+ years primary care NP experience.
Comfortable with technology and EMRs; strong written & verbal communication.
Organized, flexible, collaborative, and eager to build exceptional programming.
Experience in IBD management, infusion therapy or specialty clinic settings.
Clinic operations/quality improvement experience (policy writing, metrics/KPIs, audit cycles).
Preceptorship/mentorship experience.
Manage wound care and provide patient follow-up instructions.
Ability to work effectively in a fast-paced environment and as part of a collaborative team.
Attention to detail
EDUCATION
Current registration with the College of Nurses of Ontario as NP (PHC or Adult) in good standing, with appropriate PLP.
