Sanguen Health Centre partners with other agencies to provide health system navigation, access to Primary Care, at partner agencies, at street locations and the Primary Care Bus across the Region of Waterloo.

Responsibilities:



Works as part of the community based, interdisciplinary primary health care team. Works at the top of scope of NP practice. Demonstrates safe, quality practice. Provides assessment and clinical intervention within nursing scope, client education and assists practitioners as requested. System navigation related to healthcare and housing support for individuals who are precariously housed. Assesses and triages clients to prioritize urgent needs.

Qualifications:



Demonstrated experience working with people who use drugs and people who are vulnerable, living in poverty, and/or experiencing homelessness. Current certification as a Nurse Practitioner in the Province of Ontario with CNO membership. Minimum of 3 years' nursing experience - in a community setting, or combination of community and hospital or public health settings. Experience in primary care within a street/outreach nursing setting is an asset. Experience working with racialized populations, Indigenous peoples, people who engage in sex work, prisoners/ex-prisoners, people with mental health challenges, and people who have experienced trauma, is an asset. Thorough knowledge and proficiency in current nursing principles and practices especially regarding Mental Health and Addictions. Knowledge and understanding of substance use and overdose response. Ability to work collaboratively within and across teams. Understanding of Anti-oppression principles and ability to work effectively with people of various cultural, age and economic backgrounds. De-escalation and conflict resolution skills preferred. Ability to work effectively within a multidisciplinary team. Computer proficiency with electronic charting. Maintain a valid driver's license, access to a reliable personal vehicle and adequate insurance for work-related travel.

This position also requires:



A clear driver's abstract. A satisfactory vulnerable sector check. A valid G-class license and vehicle insurance. Willingness to be trained to drive company vehicles (as needed only). Professional liability protection to practice as a Nurse Practitioner in Ontario. Have emergency first aid (or equivalent) and BLS (Health Care Provider level preferred). Hours and Location of Work



Working hours will be approximately 20 hours per week, daytime and early evening hours.

Shifts will include days, evenings and weekends, as well as scheduled on-call hours. Shifts will occur at partner agencies, at street locations and the Primary Care Bus across the Region of Waterloo.

The successful candidate must be able to travel to and between these locations by personal vehicle. Mileage will be reimbursed for travel between work locations within a shift.

Compensation and Other Perks



This is a salaried position of $60,000 - $65,748.80 annually (equal to ($57.69-$63.22/hour) depending on the candidate's experience. 4 weeks' vacation (pro-rated to FTE)which accrues biweekly. A comprehensive Employee Benefits package upon successful completion of the probationary period. 15 paid sick days per calendar year.
