As a Nurse Practitioner, you will play a key role in delivering comprehensive care, promoting wellness, and ensuring the highest standard of patient-centered service.

Responsibilities and Duties

Conduct comprehensive health assessments and develop individualized care plans for residents. Diagnose and manage acute and chronic illnesses in collaboration with the interdisciplinary healthcare team. Prescribe medications and order diagnostic tests as permitted within the scope of practice. Provide education, counseling, and support to patients and their families. Monitor treatment plans and evaluate outcomes, adjusting care as needed. Collaborate with physicians, nurses, and other healthcare professionals to ensure continuity of care. Maintain accurate, up-to-date clinical documentation and comply with all legal and ethical standards. Promote health, prevention, and wellness initiatives within the facility. Participate in staff training and contribute to quality improvement initiatives.

Qualifications

Bachelor's Degree in Nursing (Master's Degree in Nursing or equivalent NP program preferred) Current registration in good standing with the

College of Nurses of Ontario (CNO)

as a

Nurse Practitioner

Minimum of 1 year of experience in a long-term care, primary care, or community health setting (preferred) Excellent clinical judgment, communication, and interpersonal skills Strong organizational and time-management abilities Must be available for

weekend shifts
