Nurse Practitioner (NP) Primary Care x 1 vacancy

Dr. Preutu Family Medicine Clinic (Hamilton Family Health Team)

At Dr. Preutu Clinic, we are looking for a Primary Care Nurse Practitioner to join our team.

Location: Crown Point Family Health Centre, 67 Kenilworth Avenue North, Hamilton, ON, L8H 4R6

Contract Type: permanent, part-time, 2 days per week

Salary: $55- $65 per hour + benefits

*Newly qualified or experienced applicants are welcome to apply.

OR

Contract Type: temporary locum, part-time, 1 day per week on Fridays, 6-month contract

Salary: $80 per hour self-employed

*For the locum position, a minimum of 1 year experience as a NP in a primary care setting is required. Must be able to work independently.

OR

Temporary Locum with view to permanent position, on mutual agreement.

Start date: December 1, 2025

About the Job

We are seeking a passionate Nurse Practitioner to join our dynamic team.

The ideal candidate will work closely with the family physician to provide primary health care services to patients. The main duties and responsibility will include the following:

Health maintenance: Including the care of at-risk populations; wellness exams; pap tests and breast exams; prenatal care; well-baby/child visits and immunizations. Conduct periodic examinations including use of an age/sex specific recommended preventative interventions and risk assessments; home visits for the frail elderly/geriatric patients with multiple medications and administration of immunizations.

Initiate and conduct a broad range of counselling and educational services including birth control, hypertension, and menopause, smoking cessation, osteoporosis, sexuality, breastfeeding and parenting.

Chronic Disease Management: Including but not limited to diabetes, hypertension, cardiovascular, arthritis, asthma, schizophrenia and patients with disabilities.

Assessment and Treatment of Intermittent Illness: Including but not limited to respiratory illnesses, ear and throat, gastrointestinal, genital urinary infections and dermatological.

Required Certification / Licensure

Current registration with the College of Nurses of Ontario with registration to practice as a Nurse Practitioner.

Nurse Practitioner Certification in the specialty of Primary Care

Required Education

Bachelor of Science in Nursing (BSN)

Master or Diploma - Primary Health Care Nurse Practitioner (PHCNP)

Required Skills

Must have experience working with general computer systems, Microsoft Office, Electronic Medical Records Software. Experience using OSCAR or Avaros is considered an asset.

Must demonstrate exceptional health assessment and clinical skills across the lifespan.

Excellent organizational skills and attention to detail.

Excellent verbal and written communication skills.

Ability to prioritize and manage multiple tasks efficiently.

Ability to work co-operatively as a member of an interprofessional team within the Hamilton Family Health Team.
