As the Attending Nurse Practitioner, you will be a primary care provider for residents and a leader within the LTC home. Reporting to the Medical Director/Director of Care, you'll collaborate with residents, families, and the interdisciplinary health care team to deliver high-quality, holistic care. Your role combines direct clinical practice, leadership, education, and advocacy to ensure the well-being of residents and the advancement of best practices in elder care. The Attending NP is a primary care provider to residents and works within her/his scope legislative scope of practice as described and outlined by the College of Nurses of Ontario.
Key Responsibilities:
Provide comprehensive, person-centered primary care to residents, including health assessments, diagnoses, medication management, and care planning.
Collaborate with residents, families, and the health care team to develop, implement, and evaluate individualized care plans.
Support smooth transitions of care, including hospital discharges and palliative/end-of-life care.
Order, interpret, and follow up on diagnostic tests within NP scope of practice.
Lead outbreak management, palliative care, and other clinical initiatives in the home.
Mentor, coach, and provide education to nursing staff and interdisciplinary team members.
Contribute to quality improvement, research, and evidence-based practice initiatives.
Participate in committees (e.g., Medical Advisory, Quality, Palliative Care) to enhance resident care.
Promote safe, ethical, and professional practice in line with College of Nurses of Ontario standards.
Qualifications:
Current registration with the
College of Nurses of Ontario
as a Nurse Practitioner (Primary Health Care/Adult).
Minimum
1-2 years of clinical experience
with elderly or chronic care residents;
3+ years in LTC
is an asset.
Post-graduate training in
gerontology
or
palliative care
(asset).
Strong leadership, communication, and problem-solving skills.
Ability to mentor staff and foster a team-based environment.
Proficiency in
PointClickCare
and computer literacy.
High adaptability, resourcefulness, and compassion in resident care.
Understanding of Ukrainian language and culture is an asset.
