Position Summary:




The Nurse Practitioner (NP) at Sweetgrass Health Centre will provide comprehensive primary health care to clients across the lifespan. Emphasizing health promotion, disease prevention, and chronic disease management, the NP will work collaboratively within a multidisciplinary team to provide culturally safe, holistic care. This includes the integration of both traditional and Western approaches, ensuring services are responsive to the needs of Indigenous individuals and families within the service area.This role includes providing care to urban Aboriginal populations in Cochrane, Kapuskasing, and Hearst, as well as partner First Nations communities such as Taykwa Tagamou, Wahgoshig, and Constance Lake. The NP will be an integral member of our approved Indigenous Interprofessional Primary Care Team (IIPCT), with a focus on best practices in community health and the incorporation of traditional teachings and cultural values.

Key Responsibilities:



Deliver primary care services including health assessments, diagnosis, treatment, follow-up, and evaluation. Manage chronic conditions such as diabetes, hypertension, COPD, and mental health concerns in collaboration with clients and their families. Prescribe medications and order diagnostic tests within scope of practice and regulatory standards. Perform procedures within the NP scope and provide referrals as needed. Provide trauma-informed mental health support and coordinate care with mental health providers. Incorporate traditional healing methods by collaborating with Elders, Knowledge Keepers, and Traditional Healers. Promote and practice culturally safe, client-centered care grounded in Anishinaabe values and teachings. Lead and participate in health education, promotion, and community-based wellness initiatives. Facilitate workshops, community presentations, and group health programs. Participate in interdisciplinary case conferences, care planning, and team meetings. Maintain complete, accurate, and timely clinical documentation using electronic medical record (EMR) systems. Stay current on best practices in Indigenous and primary health care through ongoing professional development and cultural competency training.

Qualifications:



Master's degree in Nursing or completion of an accredited Nurse Practitioner program. Registered and in good standing with the College of Nurses of Ontario (CNO) as a Nurse Practitioner - Primary Health Care. Current certification in CPR and First Aid. Minimum of 2 years of experience working in primary care, community health, or Indigenous health settings is preferred. Knowledge of the health and social issues affecting Indigenous communities, including social determinants of health and historical trauma. Demonstrated ability to provide culturally respectful and relationship-based care. Skilled in clinical decision-making and comprehensive care planning. Proficient with electronic medical records and clinical documentation tools.

Additional Requirements:



Knowledge of Anishinaabe culture, language, and healing practices is a strong asset. Valid Ontario driver's license and reliable vehicle. * Clear Criminal Record Check including Vulnerable Sector Screening.
