About the role:




The Nurse Practitioner (NP) with the Nurse Led Outreach Team uses advanced practice clinical skills to provide direct clinical care, consultation and treatment in the area of Geriatrics to support the acute care needs of residents in LTCHs. They are also to provide education, mentoring and coaching to front line staff to help build capacity for the LTCH within the NYSS catchment area. Additionally, the NP participates in research and quality improvement activities.


The successful candidate will be motivated, self-directed and a key member of a dynamic interdisciplinary team with defined roles focused on quality patient care and patient flow. The successful candidate will be responsible for providing quality care to residents with diverse care needs in long-term care.




The NP collaboratively provides expert clinical care, mentorship and support to primary clinicians, nurses, and other members of the health care team to enhance clinical best practices for the elderly in LTC with the goals of:




Reducing hospital admissions and emergency department transfers for conditions treatable in LTC Shortening length of stay for residents in acute care who can safely be transferred back to LTC with appropriate support from clinical teams (Primary care providers & NP) Assisting with increasing capacity of LTC staff to detect and manage acute changes in residents' conditions

What you bring:



Master's Degree in Nursing/Nurse Practitioner Stream; or Master's Degree in Nursing with clinical expertise in Gerontology; or Master's in Nursing/Master of Science in Nursing with successful completion of a post Masters Acute Care Nurse Practitioner Certificate program or commitment to pursue within a specified timeframe Registered Nurse in the extended class with the College of Nurses of Ontario Member of the Nurse Practitioner's Association of Ontario Gerontology Nursing Certification preferred BCLS required Minimum 3 years' experience in advanced practice nursing in geriatrics/eldercare and specialty-related clinical experience as a Registered Nurse Must possess own vehicle, a current Ontario Driver's Licence and a clean driving record Ability to maintain a private and secure home office Demonstrated commitment to, and understanding of the principles of diversity, equity and inclusion Demonstrated ability to work collaboratively in an inter-professional team environment, conveying a positive and professional attitude Recent experience in a leadership role within an inter-professional team Knowledge and understanding of the health care system, LTCHs and care options for the elderly Excellent capacity building, leadership and facilitation skills Demonstrated teaching skills Demonstrated program development skills and experience Demonstrated commitment to excellence in patient/family driven care Good working knowledge of Electronic Health Care Documentation Superior interpersonal and written and verbal communication skills Bilingualism in French/English an asset Demonstrated superior organizational, administrative, and problem-solving skills Excellent computer literacy - Microsoft Programs e.g. Outlook, Word, Power Point Models behaviour that is aligned with the values of Southlake Health
