Job Description

:

I am family physician providing medical care to residents in two long-term care homes in Hamilton, ON. I am seeking a dedicated and experienced Nurse Practitioner (NP) to join my team in providing high-quality medical care to residents in two long-term care (LTC) homes in Hamilton, Ontario. This is a part-time position ideal for an NP looking to work one full day per week (Fridays) of 10 hours, providing medical coverage and support alongside me. Occasional additional coverage may be requested during my vacation periods. There will be no call coverage duties.

Responsibilities:

Perform medical rounds at two long-term care homes in Hamilton split between two homes on Fridays. Provide comprehensive assessments, diagnoses, and treatment plans for residents. Communicate and collaborate with care teams, including nursing and support staff. Document patient care in accordance with regulatory and organizational standards. Provide coverage for the physician during scheduled absences.
Qualifications:

Current registration and in good standing as a Nurse Practitioner in Ontario. Minimum 1 year of experience as a Nurse Practitioner, preferably in geriatric care or long-term care settings. Strong clinical judgment, communication, and organizational skills. Ability to work independently and manage time effectively across two sites. Experience with PointClickCare EMR preferred.
Schedule:

Fridays only, 10-hour shift, split between two LTC homes in Hamilton.
Compensation:

$80/hour, salaried. No benefits.
