Perennial delivers integrated primary care and wellness services directly within retirement residences, combining on-site and virtual care to meet seniors where they are. By proactively anticipating the needs of older adults, we improve quality of life, reduce unnecessary hospital visits, and ease the strain on the public healthcare system.

Today, we are active in over 29 retirement residences across Ontario, Saskatchewan, Alberta, and Manitoba, with continued plans for growth and expansion.

Our interdisciplinary team including physicians, nurse practitioners, registered nurses, physiotherapists, occupational therapists, dietitians, and massage therapists works together with one clear mission: to keep residents healthy, active, and comfortable in their own homes.

About The Role



We are seeking a Full-Time Nurse Practitioner Lead (NP Lead) to join our leadership team, with a start date between December 2025 and January 2026. This hybrid role is based out of Toronto or Oakville, Ontario, with 3 days in-office and 2 days virtual each week.

As the NP Lead, you will play a dual role:

Clinical Leadership

- Oversee and support a team of part-time Nurse Practitioners across Ontario, Alberta, Manitoba, and Saskatchewan. You will provide mentorship, ensure clinical best practices, coordinate scheduling, and help standardize care delivery across provinces.

Clinical Practice

- Provide virtual patient care coverage as a practicing NP, ensuring consistent, high-quality access to care across our residences.
This is an excellent opportunity for a senior NP who is passionate about clinical leadership, team development, and senior care innovation, while still maintaining direct patient practice.

Responsibilities



Leadership & Management



Lead and manage a team of part-time NPs across multiple provinces. Oversee NP recruitment, onboarding, scheduling, and performance management. Provide mentorship, case consultations, and ongoing professional development. Ensure compliance with provincial regulatory requirements and Perennial's clinical standards. Collaborate with operations leadership to align staffing models with resident needs.

Clinical Practice



Provide virtual NP services for residents or staff, including primary and urgent care. Assess, diagnose, and manage chronic and acute conditions. Prescribe medications, order diagnostic tests, and develop individualized care plans. Support continuity of care through virtual coverage and cross-provincial support. Contribute to quality improvement and best practice initiatives.

Strategic Contribution



Help shape clinical protocols and models of care as Perennial expands. Provide input into cross-provincial expansion, particularly around NP licensure and scope of practice. Partner with physicians, allied health providers, and leadership on integrated care delivery.

Qualifications



Clinical Experience:

3-5+ years of NP practice.

Education:

Master of Nursing (MN) or equivalent graduate degree with a clinical focus.

Licensure:

Current registration in good standing with the

College of Nurses of Ontario (CNO)

, plus eligibility or willingness to obtain licensure in other provinces (Alberta, Manitoba, Saskatchewan).

Multi-Province Practice:

Preference for candidates already licensed in more than one province, or willing to complete additional registrations.

Skills:

Strong clinical judgment, leadership, mentoring ability, and excellent communication skills.

Passion for Seniors' Health:

Demonstrated interest in geriatrics, preventative health, and improving healthcare delivery models.

Adaptability:

Comfortable working in a hybrid role balancing leadership, strategy, and direct patient care.
