Job Summary
Southlake's Regional Cardiac Program is the 4th largest in Cardiac center in Ontario that provides care for patients requiring Cardiac surgery, Interventional Cardiology, Electrophysiology and Advanced heart failure management. The program provides tertiary and quaternary cardiac services for residents in York Region and Simcoe-Muskoka and is very proud of the clinical advancements provided to the communities served. The Interventional Nurse Practitioner (NP) is an advanced practice provider specializing in the management of patients undergoing interventional cardiac procedures. Working closely with interventional cardiologists and the broader cardiac team, the NP plays a vital role in both procedural and non-procedural settings.
This role provides clinical support during cardiac physiology studies, including assessment and monitoring of patients in both the clinic and procedural environments. The NP also coordinates and triages emergent STEMI cases, responding to calls from partner hospitals to facilitate rapid access to cardiac catheterization and interventional services.
On the inpatient and critical care units, the NP manages post-procedural care for patients recovering from percutaneous coronary interventions (PCI). Responsibilities include daily rounding, optimization of medical therapy, interpretation of diagnostics, care coordination, patient and family education, and preparation for safe discharge.
The NP ensures continuity of care from pre-procedural assessment through to post-discharge follow-up, collaborating across multidisciplinary teams to improve patient outcomes and streamline transitions of care. Additional duties may include contributing to quality improvement initiatives, participating in staff education, and maintaining documentation in compliance with institutional protocols and evidence-based practices.
Qualifications
Masters in Nursing/Nurse Practitioner Stream, or
Masters in Nursing/Master of Science in Nursing with successful completion of a post Masters Acute Care Nurse Practitioner Certificate program from a recognized school of graduate studies
Minimum 3 years relevant clinical specialty-related experience as a Nurse Practitioner
Recent experience in Geriatrics and Chronic Diseases Management
Registered in Good Standing with the College of Nurses of Ontario
Current member of RNAO
Clinical specialty organizational membership preferred
Nurse Practitioner's Association of Ontario
BCLS required, ACLS preferred
Minimum 3 years' experience as advanced practice nurse in specific clinical specialty preferred
Demonstrated ability to work in a collaborative practice model with interprofessional team.
Demonstrated ability to use initiative and work autonomously utilizing critical thinking skills, and advanced decision-making, problem recognition and problem solving skills.
Experience in clinical nursing, and interprofessional leadership and education.
Experience with program development, quality improvement initiatives, and program evaluation.
Effective organizational, interpersonal and communication skills, and demonstrated negotiation and conflict resolution skills.
Demonstrated critical thinking skills, autonomy and the ability to preform under pressure in a fast-paced, highly specialized cardiovascular environment

Primary Responsibilities
Engages in direct and in-direct advanced clinical practice, education, research and consultation within an identified clinical specialty area to ensure the highest quality of care is provided to patients/caregivers.

Triage and coordinate emergent interventional cases, particularly STEMI alerts from external facilities.
Assist in cardiac physiology studies and support invasive and non-invasive cardiac diagnostics.
Perform comprehensive assessments and manage acute and chronic cardiovascular conditions in the inpatient setting.
Provide post-procedure care, including monitoring, medication titration, and discharge planning.
Educate patients and families on cardiac conditions, procedures, medications, and lifestyle modifications.
Collaborate with interventional team, critical care staff
