Position Summary

The Nurse Practitioner (NP) (Extended Class) uses advanced clinical skills to provide direct clinical care, education, consultation and treatment in the area of medicine, surgery, geriatrics and kidney care to support the care needs of patients in Medical, Surgical, ALC, Couchiching Health at Home, and Kidney Care. The NP is the most responsible provider for selected inpatients and participates as an active member of the multi-disciplinary care team. The NP will network to establish relationships with both internal and external partners including Long Term Care Homes, community organizations, physicians and acute care hospitalists, to enhance the circle of care for patients and their families. The NP is responsible for identifying their care parameters, not exceeding scope of practice established by the College of Nurses of Ontario, and for referring patients outside their scope of practice to the most appropriate practitioner. Responsibilities will include: program planning, implementation and evaluation, project management, planning and management of resources, utilization management, safety and risk management, and creating a supportive and healthy working environment.

PRIMARY RESPONSIBILITIES:

  • Consult as needed with the Hospitalists, Specialists, Community Primary Care Practitioners and all members of the Interdisciplinary Team
  • Within a model of collaborative practice, the NP) uses advanced theoretical knowledge base and clinical competence and skill to assess, plan, provide and facilitate care across a continuum of care as assigned
  • Functions in the role as Most Responsible Provider (MRP) for select patient groups
  • Assess patients/families through multiple strategies, including comprehensive admission history and interview, advanced physical examination, interval assessments, and the interpretation of diagnostic testing to determine need for medical, nursing, or other care
  • Utilizes diagnostic reasoning and decision-making process to formulate a list of hypotheses, determine the need for, order, and interpret diagnostic testing, and formulation of a medical diagnosis within the specialty scope of practice and consistent with practice guidelines
  • Communicates significant clinical findings to consulting physicians and facilitates ongoing communication of patient progress and plan of care to the health care team
  • Initiates/orders, tailors and evaluates/discontinues selected medical and nursing therapy (including pharmacologic therapy) within the specialty scope of practice and consistent with practice guidelines and established medical directives if needed for NP to optimize the plan of care
  • Performs specialty-specific diagnostic and therapeutic interventions in accordance with scope of practice, approved medical directives for controlled acts and established practice guidelines
  • Facilitates patient care by consulting with the patient/family and inter-professional patient care team to formulate, initiate, co-ordinate and evaluate a comprehensive and collaborative patient centered plan of care, ensuring effective and efficient use of resources
  • Writes/dictates consultations, progress notes, and discharge summary reports
  • Evaluates patient care outcomes and nursing practice issues to ensure the highest possible quality of care.
  • Supports palliative care, Medical Assistance in Dying (MAID) for inpatients
  • Supervises/Precepts Nurse Practitioner students and other interdisciplinary students as required in their clinical experience and evaluates their performance
  • Provides clinical nursing leadership to promote professional development to assist nursing staff in the application of knowledge/theory to practice through role-modelling and coaching, and by participating in formal staff education programs in collaboration with the program leadership team
  • Provides leadership in the critical analysis and review of literature and the application of research findings to practice through the development and implementation of clinical practice guidelines and standards of care
  • Identifies and implements strategies to translate and interpret research findings and theoretical frameworks into practice
  • Facilitates the utilization of research by nursing and inter-professional staff and contributing to knowledge base through presentation or publication
  • Supports program planning, implementation and evaluation as well as project management
  • Participates in a supportive and healthy working environment
  • Performs other duties as may be assigned by program leadership
  • It is a requirement that the NP will work a variety of shift types including days, evenings, weekends and holidays
Qualifications

Education:
  • Current registration with the College of Nurses of Ontario as a Registered Nurse - Extended (Nurse Practitioner) is required.
  • BScN required, Master\'s Degree in Nursing preferred.
  • Current registration in the Nurse Practitioner Association of Ontario (NPAO) is required. Registration with the Registered Nurses Association of Ontario (RNAO) is preferred.
  • Current BCLS required.
Experience:
  • Minimum three (3) years clinical experience in advanced practice nursing in adult medicine/surgery, geriatrics preferred.
  • Acute Care Nurse Practitioner Certificate preferred. Gerontology Nursing Certificate preferred.
  • Additional education relevant to geriatrics an asset.
  • Advanced knowledge of the nursing process, the consultation process, program planning and development, research methodology, discharge planning and community resources.
  • Experience with the development of Medical Directives, care pathways, and clinical practice guidelines to support clinical practice
  • Experience in management and treatment in wound care an asset
  • Experience in Safety and Risk management

  • Ability to work collaboratively with the Interdisciplinary and Physician Team.
  • Must have a valid drivers license and reliable vehicle to provide client care in the community
