Are you an experienced Nurse Practitioner (NP) with a passion for rural healthcare and palliative care? Do you have an interest in ensuring healthcare professionals are supported in best practice? Do you have experience in care coordination, advance care planning, assessment and consultation and you're looking for the opportunity to expand on this experience? Then the position of NP with the North Zone Palliative Care team could be for you. You will serve client, families and primary care providers across the acute care, community and long term care, in a primarily rural context. Focusing on the care of patients supported by the Palliative Care Team, with an emphasis on Enhancing Care in the Community, we support people experiencing chronic illness, frailty and palliative conditions using an early palliative approach and a holistic model of care. This role will allow you to apply your experience within an interdisciplinary care team to provide advanced practice nursing care. You will use your clinical knowledge to perform comprehensive health assessment, interventions, and diagnose health conditions. You are empowered and accountable to establish and maintain effective working relationships with internal and external stakeholders and work collaboratively with clients and families as partners in care. Your leadership skills, knowledge of resources, as well as participation in quality improvement activities will ensure best practice and quality client care. This position is negotiable to work out of communities in North Zone - Areas 3, 4, or 5. Travel within North Zone will be required.

Description:

The Palliative Care Nurse Practitioner (PCNP) is a self-directed practitioner who directs, provides, and evaluates patient care based on advanced practice nursing and medical management knowledge and skills. The PCNP has a greater breadth, depth and complexity of application of nursing knowledge, skills, attitudes, judgement and performs additional activities beyond the Registered Nursing practice (CARNA, 2011). This position functions as an integral member of the Palliative/End of Life Care portfolio by combining the roles of practitioner, teacher, consultant, leader, and researcher through integrating interdisciplinary clinical practice, nursing staff development and research activities. The PCNP helps to improve frontline nursing practice and promote advanced practice nursing within the palliative care nursing consultation team.

Required Qualifications:

Successful completion of an approved Nurse Practitioner program at a minimum of a Master of Nursing level or equivalent. Active or eligible for registration as a Nurse Practitioner with the College and Association of Registered Nurses of Alberta with a practice permit indicating appropriate stream of practice. Experience within Continuing Care streams of Home Care, Designated Supportive Living, and/or Long Term Care Willingness to complete additional education and/or certifications specific to role.
Additional Required Qualifications:

Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Minimum of three (3) years' experience. Must have experience using Microsoft applications, Meditech and Connect Care applications
