Victoria Gardens Long Term Care is seeking a dedicated
Nurse Practitioner (NP)
to join our interdisciplinary team. This is a unique opportunity to provide resident-centered care in a supportive environment while advancing nursing services and dementia care practices.
About the Role
As a Nurse Practitioner, you will function as a practitioner, consultant, educator, and advocate. Practicing autonomously within the full scope of primary health care, the NP collaborates with physicians, the Director of Care, and community partners to deliver high-quality care and lead nursing initiatives. Reporting to the Administrator, the NP works closely with residents, families/caregivers, and the health care team to develop, implement, and evaluate individualized care plans. The NP also provides leadership and mentorship to enhance the clinical knowledge and skills of team members.
Responsibilities
Diagnose residents and develop individualized care plans
Order and interpret diagnostic tests; prescribe medications and treatments
Provide clinical leadership to RNs, RPNs, PSWs, and other care team members
Collaborate with attending physicians and external agencies
Champion quality dementia care and celebrate positive resident moments
Assist residents during admissions and transitional care
Complete nursing documentation and ensure medication reconciliation
Implement resident-focused programs and policies with the Director of Care and Quality Committee
Deliver education and in-service training to clinical staff
Uphold the Residents' Bill of Rights and regulatory standards
Perform other duties as assigned
Qualifications
Registered Nurse with Extended Class (CNO)
Completion of Ontario Primary Health Care NP Certificate Program
Minimum 3 years of experience in primary health care (preferred)
Experience in long-term care, geriatrics, or dementia care (preferred)
Proficiency with PointClickCare (preferred)
Membership in NPAO and RNAO
Strong leadership, communication, and interpersonal skills
Knowledge of nursing process and care planning
Demonstrated empathy, professionalism, and confidentiality
Skilled in conflict resolution and team collaboration
Employment Requirements

Two supervisory references

Pay: $68.00-$76.00 per hour
