Role:

Nurse Practitioner (Part time 0.8FTE)

Reports to:

Manager, Clinic Operations

Description:

The Nurse Practitioner will function in collaboration with members of the Guelph Family Health Team, Physicians, patients, families, and external partners. The NP successfully functions in an expanded role demonstrating expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and promote wellness. The NP utilizes a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for patients, involving collecting and interpreting data, determining a medical diagnosis, and treating complex responses of individuals and families to actual or potential problems. The NP performs quality improvement and administrative activities and participates in policy and procedure development.

Other key components of the role include performing research activities; delivering, coordinating, and participating in education and professional development activities/programs; performing activities to further develop and strengthen the profession; acting as a resource and serving as a consultant to individuals and group within the professional community.

Position Responsibilities:

Clinical



Performs and documents a comprehensive health history and physical examination. The assessment includes psychosocial, spiritual and cultural assessments that may impact patient care needs. Identifies and orders diagnostic tests and procedures utilizing approved medical directives as needed Collects, interprets and analyzes data from multiple sources and uses this data to formulate a medical diagnosis Develops, implements and documents the plan of care for the patient. Prescribes pharmacological and non-pharmacological therapies Evaluates and revises plan of care based on response of patient/family. Initiates interventions for the purpose of stabilizing the patient in urgent and emergent situations. Coordinates the interdisciplinary plan of care for patients by liaising with members of the interdisciplinary team in order to facilitate the efficient movement of the patient through the health care system. Collaboratively conducts clinical investigations in order to monitor and improve patient care practices.

Non-Clinical



Supports leadership in the development, implementation, and evaluation of quality management programs. Provides insights in the development, implementation and evaluation of policies and procedures and care delivery models. Supports facilitation of the development of standards, protocols and clinical pathways. Participates and supports leadership on committees. Disseminates nursing knowledge through presentation or publication at local, regional, national and international levels. Maintains links with academic institutions by collaborative projects/cross appointments. Contributes to the development/implementation and evaluation of patient education programs. Conducts educational teaching sessions with individuals, groups of patients and/or families with a focus on health promotion, disease prevention and impact of illness experience on patient and family. Facilitates professional development of team members. Mentors staff during the provision of their direct care activities. Serves as an educator and clinical preceptor for nursing students in nurse practitioner programs. Encourages, supports and facilitates the development of researchable questions in clinical practice. Analyze - literature and apply research finding to practice. Facilitate the use of evidence-bases practice for all team members. Serves as a consultant in improving care and nursing practice. Provides consultation for professional practice issues related to nursing. Performs other duties consistent with the job classification as required.

Qualifications

:

A minimum of 2 years nursing practice experience as a Nurse Practitioner, with experience in all age groups is required Current Registration with the College of Nurses of Ontario in the Extended Class Completion of a master's degree in nursing is an asset Ability to function in a full scope NP role in a collaborative and interdisciplinary primary care setting Strong health assessment and diagnosis skills Self-directed individual with strong team skills High degree of flexibility
