We work with our 11 First Nation community partners and one Indigenous Friendship Centre in the provision of primary health care, traditional healing, mental wellness and addictions, home and community support services, and health promotion services.
The opportunity:
We are seeking a compassionate and skilled Nurse Practitioner (Float, Virtual) to join our primary health care team, providing care across all communities and sites served by Maamwesying North Shore Community Health Services Inc.. As a key member of Maamwesying North Shore Community Health Services Inc, and an integral part of an interdisciplinary health care team, the Nurse Practitioner (Float, Virtual) supports access to primary care by providing flexible, remote coverage across Maamwesying North Shore Community Health Services Inc. sites. This position is designed to cover provider vacation and absences while ensuring timely virtual follow-up on referrals, results, and chronic disease management. The Nurse Practitioner (Float, Virtual) works collaboratively with the Primary Care Team to enhance continuity of care, complete preventative health reviews, and support best practices in virtual clinical operations.
What you need to bring with you:
A Master's in Nursing Degree and valid certificate as a Nurse Practitioner, preferably with a Primary Health Care Focus
Bachelors of Science in Nursing degree
Current registration in good standing as a Nurse Practitioner with the College of Nurses of Ontario (CNO)
Membership with professional associations (e.g., RNAO)
Ability to provide virtual primary care, including episodic appointments, medication renewals, and urgent result follow-ups
Flexibility to offer remote coverage during provider absences, vacation, or leave
Strong skills in virtual documentation, including forms, genograms, risk assessments, and EMR charting
Commitment to supporting continuity of care by updating medications, ordering investigations, and coordinating follow-ups
Experience conducting virtual health reviews to ensure preventative screenings are current
Ability to manage referrals and track follow-up with specialists or agencies
Experience monitoring patients with multiple comorbidities in collaboration with the care team
Knowledge and skills in chronic disease management, aligned with clinical directives
Willingness to participate in developing and implementing clinical policies, procedures, and best practices
Enthusiasm for contributing to virtual education, training, and quality improvement initiatives to enhance patient care and outcomes
Strong organizational and multitasking skills to manage multiple projects and programs
Competence in conflict resolution
Knowledge of Anishinabek health and traditional health practices
Excellent oral and written communication skills
Police Record/Vulnerable Sector Check

What to expect?
Given the traditional practices of Indigenous people, from time-to-time exposure to smoke from the burning of sacred medicines such as tobacco, sweet grass, sage or cedar may occur.
