Emergency preparedness is a critical aspect of CCH operations. It is essential that all staff and physicians receive training and understand their roles and responsibilities in this field.


Summary of responsibilities


Reporting to the Manager of Withdrawal Management, Crisis and Mobile Response Services the Nurse Practitioner (NP) works collaboratively with health care professionals from primarily Community Withdrawal Management Services as well as CAMHS and across the hospital to assist in the provision of care for clients who are experiencing substance use health, addiction and/or mental health issues. The NP coordinates client care by working collaboratively with our interdisciplinary teams to assess, plan, implement, and evaluate clinical interventions.




The NP will demonstrate autonomy while assuming accountability and the authority to independently communicate a diagnosis, prescribe drugs and order a range of diagnostic tests as per approved hospital directives and the College of Nurses. Provision of care includes assessment, diagnosis, counselling, screening, referral, education, treatment and follow-up. The NP will act as a liaison and consultant to other health care providers and regional partners regarding patient care and health services when needed. They will provide leadership in the transfer of knowledge into practice in nursing and related fields appropriate to the clinical focus.


Requirements of the job


Current Registration from the College of Nurses of Ontario in the Extended Class and Licensed NP Baccalaureate Degree in Nursing from a recognized University, A minimum of three years of experience in working with clients who are experiencing substance use health, addiction and/or mental health related issues Experience working with interprofessional health care teams Demonstrated ability to manage numerous and diverse activities, organizing and prioritizing appropriately to meet demands and expectations Strong advocate for patient- and caregiver- centered care Ability to apply critical thinking to patient care Demonstrated knowledge and understanding of legal and ethical issues Advanced knowledge of the consultation process as they relate to individuals, groups and/or systems Excellent writing skills Knowledge of the regional health care system in the area of the discipline/specialty Demonstrated regular, punctual attendance Demonstrated commitment to patient safety Computer literacy (MS Office, Excel, Outlook) Functional level in English (oral and written)
Preferred:


Membership with RNAO or the Nurse Practitioner's Association of Ontario Functional level in French (oral) an asset (and testing completed)
