The NP works in collaboration and consultation with physicians, coordinators, and other members of the inter-professional healthcare team to prevent avoidable Emergency Department visits and hospitalizations and to support transitions to and from the hospital when needed. The NP will link the hospital and the LTC homes to ensure the most appropriate and efficient clinical care provision. The NP will also support NLOT program development and direction that is agile and iterative, reflective of the dynamic nature of resident care needs and evolving Ontario Health and Ministry of Long-Term Care priorities.Responsibilities:
