Please ensure that you have provided all pertinent information necessary to be considered for this vacancy by the close date.Reporting to the Clinical Manager of the Nurse-Led Outreach team (NLOT), the Nurse Practitioner (NP) role in the Nurse-Led Outreach Team program is primarily responsible for providing clinical care to support residents living in Long-Term Care (LTC) homes within a specific sub-regional boundary. The NP works in collaboration and consultation with physicians, coordinators, and other members of the inter-professional healthcare team to prevent avoidable Emergency Department visits and hospitalizations and to support transitions to and from the hospital when needed. The NP will link the hospital and the LTC homes to ensure the most appropriate and efficient clinical care provision. The NP will also support NLOT program development and direction that is agile and iterative, reflective of the dynamic nature of resident care needs and evolving Ontario Health and Ministry of Long-Term Care priorities.Responsibilities:

• Practice in accordance with the Regulated Health Professions Act, Gerontological Nurses Association (GNA) Standards, The Long-Term Care Homes Act, the Practice Standards set by the College of Nurses of Ontario and standards set out for the Nurse Practitioner
• Assist in avoiding hospital admissions for current LTC residents who have new or increasingly complex medical or specialized equipment needs which cannot currently be accommodated in their LTC home
• Support placement of eligible high-needs designated Alternate Level of Care (ALC) patients from hospitals into LTC
• Enable the admission of community members with specialized needs to LTC homes
• Assist in reducing non-urgent transfers to the region\xe2\x80\xb2s Emergency Departments
• Provide capacity building for LTC staff to develop and enhance competency for acute treatment protocols
• Provide assessment, intervention, monitoring and follow-up for acute and chronic illnesses
• Provide leadership and promote excellence in patient-centred nursing care through expert practice, education, consultation and research
• Actively participate in a Regional NLOT Community of Practice
• Provide input and support in the development and implementation of integrated hospital-community-LTCH clinical pathways
• Develop linkages with external partners as appropriate to ensure continuity of care
• Address medication reconciliation issues upon discharge from the hospital
• Utilize the Clinical Information System
• Utilize the protective equipment provided and identifies and reports hazards

Requirements:

• Master\xe2\x80\xb2s Degree in Nursing/Nurse Practitioner (MN-NP) or Master of Science in Nursing with successful completion of a post-Masters Acute Care Nurse Practitioner Certificate Program from a recognized school of graduate studies required
• Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care) required
• Must have completed a CNO-approved controlled substances education
• Must have professional Liability Protection (RNAO or equivalent)
• Current Basic Cardiac Life Support (BCLS) certification required
• 3 years of demonstrated experience as a Nurse Practitioner required
• Previous experience in the LTC sector preferred
• Additional specialty education in Gerontology preferred

3-5 years of Acute relevant medicine or Primary Care experience preferred

• Demonstrated strong communication and interpersonal skills, with the ability to work collaboratively with physicians, inter-professional team members, and other stakeholders
• Ability to maintain and protect patient confidentiality at all times, demonstrating the use of discretion and sound judgment
• Ability to promote professional education and advanced knowledge through research and evaluation/quality improvement activities related to the care of patients throughout the continuum
• Demonstrated commitment to the mission and values of SHN.
• Consistent and regular attendance is required.
• Valid Ontario Driver\'s license and access to a car required
• Clinical expertise, including but not limited to: IV initiation, suturing, venipuncture, wound care, end of life care planning.
• Demonstrated clinical leadership skills to organize and coordinate the care of patients in collaboration with their caregivers
• Motivated self-starter with the ability to work independently and collaboratively as part of a team
• Demonstrated time management skills with the ability and flexibility to prioritize multiple tasks in a fast-paced, results-oriented environment
• Advanced nursing assessment skills and sound knowledge of clinical therapeutics
• Effective patient/family counselling skills
• Excellent organizational, interpersonal, communication, leadership, critical appraisal and analytical, and decision-making skills
• Demonstrated superior written and oral communication skills
• This position will work in off-site community settings providing clinical care to support residents living in Long-Term Care (LTC) homes within a specific sub-regional boundary in Scarborough
