West Carleton Family Health Team

is located in the Village of Carp within the City of Ottawa. The West Carleton Medical Centre is a large multidisciplinary clinic with over 65 staff members. We believe in utilizing our staff to the full extent of the scope and currently have multiple Nurse Practitioners on our team.

We also believe in having the most appropriate staff member completing the most appropriate work and as such we have robust administrative support for our clinicians including our referrals team, medical records team, and many efficiency processes. We fund and utilize digital solutions to support patient care, such as AI scribe, as we believe that work life balance is a high priority and digital solutions can support this. We strongly believe in supporting our clinicians through mentorship and collaboration to ensure both optimal patient care but also to decrease clinical silos.

We are presently recruiting a

Nurse Practitioner

for a

Full Time

position. The hours may be adjusted based on the candidates preference, so if you would prefer to work less than 37.5 hours per week please do not hesitate to apply.

Job Summary

: A licensed and credentialed NP provides diagnosis, treatment, consultation, and follow-up work in collaboration with other allied health professionals and physicians. The NP provides age and specialty appropriate medical care at the level of training achieved.

Compensation:

$55/hr + 15% ($8.25/hr) in lieu of benefits. Final compensation will be determined based on Education, Experience, and Funding.

Job Duties:



Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care. Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral. Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues. Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications. Maintains legible, accurate, and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals, and consultations consistent with applicable standards. Facilitates evaluation of records by physician(s), peers, and Quality and Standards according to protocols, and receives and implements constructive directives. Analyzes new knowledge gained from conferences, workshops, professional literature, or "hands-on training" and assimilates this knowledge into clinical practice. Consults with physicians and other clinical staff where situations arise beyond their scope of practice. Participates in care coordination meetings and case coordination meetings, as required. Participates in the maintenance of a healthy and safe work environment in accordance with the facility's safety programs. Participates in performance measurement and utilization management. Collaborates in the overall management of the clinic by participating in team meetings, committees, and work groups as required. Tracking and trending outcome data in collaboration with clinical and management staff. Promotes communication to develop a collaborative multi-disciplinary care team. Acts as mentor/coach/teacher for more junior staff.

Education and Experience



Mandatory Requirements Must hold a RN - Extended Class registration. Must possess skills in assessment, diagnosis and treatment of a broad spectrum of acute and chronic illnesses Must possess knowledge of health care management, counselling and patient teaching and communication Must be able to work in a multi-disciplinary team Computer skills, preferably in a comprehensive EMR and Microsoft environment. Excellent interpersonal skills; and client service orientation.

Hours of work



You will be required for 37.5 hours per week which includes 4 days and 1 evening shift (Evening shifts could be a Monday to Thursday, ending by 8 PM).
