Work Type: Temporary Part Time

Position Summary

: The

Nurse Practitioner (NP)

will utilize their full scope of practice to formulate clinical decisions and appropriately manage acute medical concerns, cardiovascular risk factors and promote wellness in the Cardiovascular Prevention and Rehabilitation Program. The NP consults and collaborates with individuals, their families, and the interdisciplinary team. The NP will provide a liaison role with Physicians and community partners regarding the clinical management of program clients. Additionally, the NP provides individual and group education to clients, families and allied health team members, as needed.


Position Requirements:



Master's prepared Registered Nurse with Nurse Practitioner certificate Registration with the College of Nurses of Ontario in the Extended Class Member of NPAO with additional professional liability coverage Minimum one year's experience in a primary care setting managing preventative care of patients with hypertension, Type 2 diabetes mellitus, smoking cessation, amongst others. Demonstrated ability to work independently with a high degree of self -direction and strong organization skills; as well as working effectively as a member of an inter -professional team Proven excellence in interpersonal and communication skills (written and oral) including conflict resolution skills Record of, and commitment to, continuing education/professional development Keyboarding skills required; familiarity with healthcare software is an asset Demonstrated commitment to the values of service, responsible stewardship, community responsiveness and respect
