Your Opportunity:
The Nurse Practitioner is an integral clinician and leader within the Integrated Home Care program. You will work to your full NP scope in a team-based model alongside Primary Care physicians for collaboration and support for complex case management. Acting as a resource for stakeholders across the care continuum, the NP plays a key role in addressing the growing demand to manage complex medical needs in the community. This includes utilizing advanced assessment and physical examination skills, diagnosing and managing common, complex, and chronic health conditions, promoting health and preventing illness or injury and assisting clients and caregivers in navigating the health care system. The incumbent will be a strong leader in the advocacy of client and family centered care. This role requires innovation and creativity and will continue to evolve to meet the needs for the population we serve. This is an ON-SITE position.
Description:
As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Required Qualifications:

Successful completion of an approved Nurse Practitioner program, with a minimum of a Master's degree. Active registration with the College of Registered Nurses of Alberta (CRNA) on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Active registration with the College of Registered Nurses of Alberta (CRNA) as a Nurse Practitioner in the appropriate stream (e.g., Family/All Ages, Adult). Completion of a Master's degree in Nursing is required. This position requires a vehicle for business use. Possession of a valid driver's license and access to a motor vehicle in good condition for work-related travel is required. Knowledge of and ability to support change management. Advanced Microsoft office skills (Power Point, Excel) with proficiency in MS Outlook and MS Teams. You will be responsible to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example, A push/pull of up to 50lbs for compression therapy on a regular basis is an expectation of this job. Learning resources and training are provided to safely perform work. Minimum of three years' experience with older adults in the community within the last five years. Experience in research and best practice implementation. Recent experience working as an NP with the older adult population. Proven experience with teaching, supervision, coaching and mentorship. Ability to collaborate with stakeholders within and outside of AHS. Proven initiative, autonomy, conflict resolution & critical thinking skills.

Preferred Qualifications:

Demonstrated excellent written & verbal skills with continuing education in the area of the older adult. Demonstrated self-directedness in organizing work prioritize & flexibility in meeting the unexpected spontaneous needs of a clinical area. Demonstrated ability to function effectively & collaboratively within a client and family centered care team of health care professionals and support staff. CNA certification in Gerontology.
