Your Opportunity:The Geriatric Nurse Practitioner (GNP) plays a dynamic role in consulting on frail individuals and advocating for equitable care for complex, co-morbid patients in acute care. Using Comprehensive Geriatric Assessments (CGA), the GNP establishes management plans based on patients\' priorities and their interactions with the healthcare system. This role suits those who can blend evidence-based practice with creative problem-solving to meet patient and family goals. The GNP collaborates with patients, families, geriatricians, clinical nurse specialists, allied health professionals, and nursing teams. The GNP is skilled in managing comorbid diseases and iatrogenic complications in frail adults within an acute care setting. This role combines nursing and medical skills to assess, diagnose, interpret, and recommend treatment for individuals living with frailty. The GNP is a self-directed practitioner who fulfills the roles of clinician, leader, educator, consultant, and researcher to create individualized management plans. This role offers opportunities to specialize based on your interests. In collaboration with site geriatricians, the primary responsibility of the GNP is to conduct CGAs for patients referred to FMC\'s acute care Geriatric Consultation Service. The GNP will recommend management strategies for comorbidities, including frailty and frailty syndromes, and may also consult, assess, and follow up with clients at other acute care sites or ambulatory clinics as needed. They will work across Inpatient, Outpatient, and Emergency Departments at FMC to provide geriatric assessments and consultations. Additionally, the GNP may participate in projects across sites, sectors, the Calgary Zone, and the province.Description:As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Required Qualifications:Successful completion of an approved Nurse Practitioner program and a minimum of master\'s degree. Registration with CRNA on the Nurse Practitioner Registrar including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:Three years of experience in a geriatric focused specialty program (e.g., specialized geriatrics, AGU, or equivalent). Three years of experience as a Nurse Practitioner. Proven leadership skills and experience in advocating for elder-friendly care and policies. Strong ability to provide expert advice and consultation to healthcare teams and leadership. Exceptional communication skills, capable of effectively engaging with patients, families, and healthcare professionals. Deep commitment to providing compassionate, patient and family-focused geriatric care. Ability to work independently and collaboratively within a multidisciplinary team. Preferred Qualifications:Current CNA certification in Gerontology.
